This medication is used for the treatment of essential hypertension. It also helps reduce the risk of death from myocardial infarction, stroke, and cardiovascular disease in high-risk patients aged 55 and older who cannot use ACE inhibitors. It is particularly effective in high-risk diabetic patients with a history of coronary artery disease, peripheral artery disease, stroke, transient ischemic attacks, or evidence of end-organ damage. Therefore, this medication improves health status not only through blood pressure control but also by reducing the risk of cardiovascular diseases.

How to take

Adult dose

Essential hypertension: It is recommended to take 40 mg orally once daily as Telmisartan, which can be effective at 20 mg once daily depending on patient needs. If necessary, the dose may be increased to 80 mg once daily. Reducing cardiovascular disease risk: It is recommended to take 80 mg orally once daily. Patients with liver impairment: Patients with mild to moderate liver impairment should be treated with caution and should not exceed 40 mg once daily.

Precautions

This medication can cause serious harm to the fetus and newborn, especially during pregnancy, so immediate discontinuation is necessary upon confirmation of pregnancy. Individuals who are allergic to this medication or its components, pregnant or nursing women, patients with severe liver impairment or bile excretion issues should not take this medication. Additionally, patients with hereditary angioedema, those with a history of angioedema while taking ACE inhibitors or angiotensin II receptor antagonists, diabetic patients, patients with severe kidney impairment, and those with lactose intolerance should also avoid this medication. Elderly patients, individuals with high potassium levels in the blood, patients with mild to moderate liver impairment, valvular heart disease, ischemic heart disease, gastrointestinal diseases, and renal vascular hypertension should use this medication cautiously and closely follow medical advice. Adverse reactions such as blood pressure drop, dizziness, angioedema, renal dysfunction, liver function abnormalities, and hyperkalemia may occur during treatment; any symptoms should be promptly discussed with a healthcare provider. In particular, immediate treatment is necessary if blood pressure drops too low or if symptoms of shock or fainting occur. Patients with impaired kidney function should have serum potassium and creatinine levels tested regularly, and although patients undergoing dialysis can use this medication without dose adjustments, their condition should be closely monitored. Patients with reduced blood volume or sodium deficiency are at risk of hypotension, so their condition should be corrected before administering this medication. Caution is advised when using this medication with other antihypertensives, as it may enhance blood pressure-lowering effects, and when taken with certain medications, there may be an increased risk of renal dysfunction or hyperkalemia, necessitating consultation with a physician. Pregnant women or those planning to become pregnant should not use this medication, and breastfeeding patients should either avoid it or discontinue nursing. Safety and efficacy have not been confirmed in children under 18, so use is not recommended. Elderly patients should be carefully monitored due to the risk of excessive blood pressure reduction leading to stroke and other complications. In cases of overdose, symptoms such as hypotension, arrhythmia, dizziness, and renal failure may occur, necessitating immediate medical treatment; this medication cannot be removed by dialysis. When stored, keep this medication out of reach of children and do not transfer it to other containers.
