Improvement of cholesterol and lipids in hypercholesterolemia (primary, familial) and mixed hyperlipidemia
This medication is used as an adjunct to diet to lower total cholesterol, LDL-cholesterol, apolipoprotein B, and triglyceride levels while raising HDL-cholesterol levels in patients with primary hypercholesterolemia, particularly heterozygous familial and non-familial hypercholesterolemia, and mixed hyperlipidemia. It is also administered as an adjunct to other lipid-lowering therapies in patients with homozygous familial hypercholesterolemia, and can sometimes be used as monotherapy. This medication is indicated for hypercholesterolemic patients where dietary and non-pharmacological interventions are insufficient. Prior to treatment, secondary causes such as diabetes and hypothyroidism should be identified and treated if necessary. During treatment, levels of total cholesterol, LDL-cholesterol, HDL-cholesterol, and triglycerides should be monitored, especially if triglycerides are above 400 mg/dL, where careful measurement is required. For patients with acute coronary events, lipid testing is recommended at the time of hospitalization, and this information is helpful for appropriate treatment decisions. This medication effectively reduces the risk of cardiovascular disease associated with hypercholesterolemia and improves lipid profiles.
This medication is administered once daily in the evening regardless of meals. The dosage range for patients with primary hypercholesterolemia is 10/10 mg to 10/40 mg daily, with the generally recommended starting dose being 10/20 mg daily. Patients needing less reduction in LDL-cholesterol can start at 10/10 mg daily. After dosage adjustment, lipid levels should be monitored after at least 4 weeks before any further dosage modifications. For patients with homozygous familial hypercholesterolemia, the recommended dosage is 10/40 mg daily, administered once in the evening. This medication is used as an adjunct to other lipid-lowering treatments (e.g., LDL Apheresis) or in cases where other lipid-lowering treatments are not beneficial. For patients with liver impairment, dosage adjustment is not required for mild hepatic impairment, but this medication is not recommended for moderate to severe hepatic impairment. For patients with renal impairment and chronic kidney disease, dosage adjustment is not needed for mild renal impairment (approx. GFR ≥ 60 mL/min/1.73㎡). For patients with chronic kidney disease and an estimated GFR < 60 mL/min/1.73㎡, 10/20 mg should be administered once daily in the evening. Caution should be taken when administering higher doses to these patients, and careful monitoring is required. There is no need for dosage adjustment in elderly patients. When co-administrating with other medications, this medication should be taken 2 hours before or 4 hours after bile acid-binding resin therapy. Patients receiving verapamil, diltiazem, or dronedarone should not exceed a dose of 10/10 mg/day. Patients receiving amiodarone, amlodipine, or ranolazine should not exceed a dose of 10/20 mg/day.
This medication is primarily used to lower cholesterol levels in the blood. When taking this medication, it may cause muscle-related issues, so if symptoms such as muscle pain or weakness occur, consult a physician immediately. Patients with liver dysfunction or liver disease should not use this medication, and regular liver function tests are essential. Pregnant or nursing women should not take this medication, and those planning to become pregnant should consult with a physician. This medication can increase the risk of muscle damage when taken with certain drugs; therefore, it is essential to inform your physician if you are taking other medications. During treatment with this medication, it is advisable to avoid certain foods like grapefruit juice. Elderly patients or those with decreased kidney function need to exercise extra caution when taking this medication. Additionally, this medication may slightly increase blood sugar levels, so diabetic patients should manage their blood sugar closely. In case of overdose, there is no specific antidote, so the prescribed dosage must be strictly adhered to, and if abnormal symptoms occur, medical attention should be sought immediately.
Allergic reactions including hives, angioedema, and rash, pancreatitis, cholelithiasis, cholecystitis, angioedema, rash, toxic epidermal necrolysis (Lyell's syndrome), Stevens-Johnson syndrome, erythema multiforme, pancreatitis, hepatitis (including chronic active hepatitis), cholestatic jaundice, hepatic fat changes, cirrhosis, acute hepatic necrosis, liver cancer, hepatic failure, chest tightness, difficulty breathing, respiratory distress, toxic epidermal necrolysis (Lyell's syndrome)
Headache, diarrhea, influenza, upper respiratory infection, myalgia, limb pain, fatigue, nausea, abdominal pain, constipation, dyspepsia, flatulence, vomiting, itching, rash, nocturia, hematuria, gastritis, atrophic gastritis, mucous stools, irritable bowel syndrome, sore throat, weakness, sensory loss, insomnia, syncope, paresthesia, increased serum creatine phosphokinase (CPK), weight loss, gynecomastia, decreased libido, sexual dysfunction (including erectile dysfunction)

This medication is sensitive to air and moisture, so it must be stored in a tightly closed container. Also, to maintain the efficacy of the medication, it should be kept at room temperature between 1 and 30 degrees Celsius.
