Improvement of essential hypertension
Essential hypertension is a condition where blood pressure is persistently high, placing strain on the heart and blood vessels. This medication helps lower blood pressure and reduce the risk of cardiovascular disease. With consistent use and management, it aids in maintaining healthy blood pressure levels. Therefore, it is used for the treatment of essential hypertension.
The recommended starting dose is 10mg once daily, taken regardless of meals. If blood pressure is not adequately controlled at this dose, it may be increased to 20mg once daily. If additional blood pressure-lowering effects are needed, it can be increased up to a maximum of 40mg once daily. The antihypertensive effect typically occurs within 2 weeks of starting treatment, with maximum effect observed around 8 weeks. If blood pressure is not controlled when this medication is used alone, thiazide diuretics such as hydrochlorothiazide may be used in combination. To improve adherence, it is recommended to take the medication at the same time every day (e.g., in the morning). In patients with the potential for intravascular volume depletion (e.g., diuretic users, particularly those with kidney impairment), it is recommended to begin treatment at a lower dose while closely monitoring their condition. <Elderly Patients> For patients aged 65 years or older, dose adjustment is not necessary. While no overall differences in efficacy and safety between elderly and younger patients were observed, increased sensitivity in some elderly patients cannot be excluded. <Patients with Kidney Impairment> The maximum dose for patients with mild to moderate kidney impairment (creatinine clearance 20-60mL/min) is 20mg once daily. <Patients with Liver Impairment> Mild liver impairment does not require dose adjustment. For moderate liver impairment, the recommended starting dose is 10mg once daily, which may be increased to a maximum of 20mg once daily. There is no experience with the use of this medication in severe liver impairment, hence its use is not recommended. <Pediatric Patients> Safety and efficacy in pediatric patients have not been established.

This medication can cause serious harm or death to the fetus and newborn, especially during the second and third trimesters of pregnancy, so use should be discontinued immediately upon confirmation of pregnancy. This medication should not be used by individuals who are allergic to it, pregnant or breastfeeding women, patients undergoing kidney dialysis, individuals with severe liver impairment, or patients with bile duct obstruction. Patients with reduced blood volume or those taking high doses of diuretics may experience hypotension; therefore, they should carefully check their condition and begin treatment accordingly. Patients with impaired kidney function, kidney transplant recipients, patients with renovascular hypertension, and those at risk for hyperkalemia require careful monitoring and regular testing when taking this medication. Patients with aortic stenosis, mitral stenosis, obstruction, or hypertrophic cardiomyopathy should also use caution when taking this medication. Dizziness may occur during treatment; therefore, operation of hazardous machinery or driving should be avoided. Mild side effects such as dizziness, headache, dyspepsia, back pain, bronchitis, increased creatine phosphokinase, diarrhea, hematuria, hyperglycemia, hypertriglyceridemia, influenza-like symptoms, pharyngitis, rhinitis, sinusitis, abdominal pain, dyspepsia, gastroenteritis, nausea, hypercholesterolemia, hyperlipidemia, hyperuricemia, arthralgia, arthritis, myalgia, rash, hair loss, pruritus, cough, epistaxis, and upper respiratory infections may occur. Rarely, angioedema or liver function abnormalities have been reported. Chronic diarrhea and weight loss associated with chronic malabsorption-like enteropathy may occur; if these symptoms develop, consult a doctor immediately. When taken with potassium-sparing diuretics or potassium supplements, blood potassium levels may increase, so caution is advised, and co-administration with lithium preparations is not recommended. Taking this medication simultaneously with non-steroidal anti-inflammatory drugs may worsen kidney function, hence hydration and renal function tests are required at the beginning of treatment. It is crucial to avoid using this medication during pregnancy to protect fetal health, and if breastfeeding, consider discontinuing the medication or stopping nursing due to potential adverse reactions in the infant. Safety in pediatric patients has not been established, and elderly patients may require dosage adjustment and caution due to increased drug concentration in the body. Overdose may result in hypotension, palpitations, bradycardia, etc., and appropriate treatment is needed based on the symptoms.
