This medication is used adjunctively during various ophthalmic surgeries such as lens implantation in cataract surgery, corneal transplant surgery, and glaucoma surgery.

How it is given

Adult administration

For cataract surgery and intraocular lens implantation, the required amount should be slowly injected into the anterior chamber of the eye via a needle or cannula before or after lens extraction. During corneal transplant surgery, after removing the corneal graft and while aligning the corneal surface with the aqueous humor, this medication should fill the anterior chamber until the graft is placed and sutured. In glaucoma filtering surgery, this medication is injected through a corneal paracentesis to supplement and maintain anterior chamber volume during trabeculectomy.

Precautions

This medication is made of sodium hyaluronate and is primarily used during ophthalmic surgeries. It should not be administered to patients who have a history of allergy to this medication’s components or to any protein-based medication. After use, various side effects may appear in the eyes, such as increased intraocular pressure, inflammation, corneal edema, corneal opacity, and bleeding. Rarely, abnormalities in the iris or opacification of the lens may occur. In patients using intraocular lenses, there is a possibility of opacification on the surface of the lens. This product contains phosphates, which have been rarely reported to cause calcification of the cornea in patients with severe corneal damage. Particularly after surgery, it is essential to closely monitor the eye pressure; if the intraocular pressure rises, appropriate measures should be taken. After surgery, it is advisable to cleanse or aspirate this medication. When performing lens extraction surgery using ultrasound, it is necessary to inject irrigation fluid between this product and the lens to create space. When cleaning surgical instruments, solutions containing quaternary ammonium compounds such as benzalkonium chloride should not be used. Since this product is a substance isolated and purified from avian cells, be aware that there may be immunological reactions or other potential risks upon administration. Furthermore, it should not be used in conjunction with other medications containing hyaluronidase. This medication is only to be administered directly to the eye, and intravenous injection is strictly prohibited. Before use, it is recommended to allow the refrigerated medication to reach room temperature for approximately 30 minutes. Caution is needed as precipitates may form when in contact with components such as benzalkonium chloride or chlorhexidine. Once opened, it should be used only once, with any remaining solution disposed of in its container. When using this medication in conjunction with air, care must be taken to ensure that air does not enter the eye, especially beneath the iris. When injecting the medication, it should be administered slowly and carefully; excessive injection may lead to increased intraocular pressure after surgery, so avoid overdose. For storage, keep this medication out of reach of children and store it in a cool, dry place away from direct sunlight, in a sealed container. To maintain quality, do not transfer the medication to other containers and take care to prevent misuse.

Side effects and when to seek care immediately

Mild side effects

Increased intraocular pressure, inflammatory reactions, corneal edema, corneal opacity, anterior hemorrhage, neovascularization of the iris, posterior iris ulcer, conjunctival adhesion failure, mydriasis, lens opacity, pain, itching, blurred vision, corneal dysfunction, opacification of the surface of the intraocular lens, corneal calcification.

Storage

This medication should be stored in a sealed container to prevent contact with air, and kept in a cool place between 2 and 8 degrees Celsius. Furthermore, care should be taken to avoid direct sunlight and freezing to ensure safe usage.
