View approved original text

This medication is used to assist in blood sugar control for patients with insulin-independent diabetes, namely type 2 diabetes. It is particularly administered when blood sugar control is insufficient with monotherapy of either Metformin or Pioglitazone and can also be used instead of combination therapy with both medications. Management of type 2 diabetes is critical and involves dietary and exercise therapy, and weight loss along with nutritional counseling is also necessary. Such lifestyle improvements are essential not only for initial treatment but also for maintaining the effectiveness of medication therapy. This medication helps in improving blood sugar control alongside these treatments.

Dosage and Administration

Adult dosage

The initial dosage of this medication is based on the patient's current therapy regimen, starting with one tablet daily and gradually adjusting the dosage based on the adequacy of the treatment response. Patients transitioning from combination therapy of Pioglitazone and Metformin may start with their existing doses. This medication is generally taken with meals to minimize gastrointestinal side effects associated with Metformin.

Precautions

This medication is used to control blood sugar and consists of two main ingredients. The first ingredient is Metformin, which helps utilize sugars effectively in the body and lowers blood sugar levels. However, this ingredient can rarely cause lactic acidosis, a serious metabolic problem where lactic acid accumulates excessively in the body, leading to a dangerous state. Lactic acidosis is more likely to occur in patients with impaired renal function, heart or lung issues, and in cases of infections or dehydration, so caution is required. Moreover, the use of this medication should be avoided in patients with liver dysfunction or those consuming excessive alcohol. The second ingredient is Pioglitazone, which aids insulin action and helps control blood sugar. However, this ingredient can exacerbate or newly induce heart failure, so patients with severe or symptomatic heart diseases should not use this medication. Additionally, this medication can cause fluid retention, leading to edema or weight gain, necessitating careful monitoring for symptoms of heart failure. This medication should not be administered to patients with impaired renal function, severe heart failure, active bladder cancer, pregnant or breastfeeding women, and certain genetic lactase deficiency disorders. Regular kidney function tests and liver function tests are recommended during medication use, as well as periodic checks for vitamin B12 levels. Furthermore, due to the risk of hypoglycemia, patients with irregular eating patterns, engaging in strenuous exercise, or those using other hypoglycemic agents should exercise increased caution. If symptoms such as sudden shortness of breath, muscle pain, weakness, or abdominal pain occur during administration, the physician should be notified immediately. Finally, this medication should be discontinued for a certain period before and after surgery or before tests that involve iodinated contrast agents. It is also safer to avoid excessive alcohol consumption. Patients and caregivers should thoroughly understand these precautions, and any abnormal symptoms during medication use should be promptly reported to healthcare personnel.
