Effect of reducing mortality risk from myocardial infarction and stroke due to reduction of essential hypertension and cardiovascular disease risk

  • Essential hypertension
View approved original text

This medicine is used to control blood pressure in patients with essential hypertension. It also helps reduce the risk of cardiovascular diseases, thereby decreasing the mortality risk from myocardial infarction, stroke, and cardiovascular disease. It is particularly effective in high-risk patients aged 55 and older who cannot use ACE inhibitors. It contributes to the prevention of cardiovascular complications in patients with a history of coronary artery disease, peripheral artery disease, stroke, transient ischemic attacks, or diabetic patients with end-organ damage. This medicine lowers the risk of major cardiovascular events and improves cardiovascular health in patients.

How to take

Adult dose

For essential hypertension, Telmisartan should be taken orally once daily at a dose of 40 mg with water, regardless of meals. Depending on the patient, a dose of 20 mg once daily may also be effective, and may be increased to 80 mg once daily if necessary. The maximum hypotensive effect appears 4 to 8 weeks after starting treatment. For the purpose of reducing the risk of cardiovascular disease, a dose of 80 mg once daily is recommended.

Precautions

This medicine should not be used during pregnancy. If pregnancy is confirmed, discontinue use immediately, as it can cause severe harm to the fetus and newborn. Patients with an allergy to this medicine or its components, pregnant or nursing women, and those with severe liver disease or biliary excretion disorders should not take it. Additionally, individuals with hereditary angioedema or a history of angioedema while taking ACE inhibitors or angiotensin II receptor antagonists, and those with diabetes or moderate to severe renal impairment should not use this medicine. Elderly patients, those with hyperkalemia, patients with impaired liver function, those with specific heart valve diseases or obstructive hypertrophic cardiomyopathy, and patients with ischemic heart disease or cerebrovascular disorders should use this medicine with caution. Patients with active gastric ulcers or duodenal ulcers are at risk for gastrointestinal bleeding, so caution is necessary. Those with renal vascular hypertension or severe renal impairment are also at risk for worsening renal function when taking this medicine. Although this medicine has the effect of lowering blood pressure, it may cause hypotension, dizziness, and fainting, so caution is needed when driving or operating machinery. Regular monitoring of blood pressure, kidney function, electrolyte levels, etc. is necessary during treatment to ensure there are no abnormalities. When taken with other antihypertensive drugs, diuretics, potassium-sparing diuretics, lithium, non-steroidal anti-inflammatory drugs (NSAIDs), etc., the risk of side effects may increase due to interactions, so it is essential to consult a doctor or pharmacist. Overdosing can cause hypotension, tachycardia, bradycardia, dizziness, and renal failure, so immediate medical attention is required. This medicine is not recommended for use in children or adolescents under 18 years of age, as its safety and efficacy have not been established. It should be kept out of reach of children, and since the tablets are sensitive to moisture, it is best to open the package just before taking it.
