Who applies the ear drops
Adults
Adult use
The initial dose may vary from 4mg to 48mg of methylprednisolone per day, depending on the symptoms of the specific disease. For treatment of acute exacerbations of multiple sclerosis, it is effective to administer prednisolone 200mg daily for 1 week, followed by prednisolone 80mg every other day for 1 month (methylprednisolone 4mg is equivalent to prednisolone 5mg). Alternate-day therapy is a regimen in which twice the daily dose is administered on one morning every other day. A single morning dose produces considerably less adrenal suppression than taking prednisolone (10mg) in 4 divided doses every 6 hours throughout the day. The initial suppressive dose is usually given for 4 to 10 days for many allergic and collagen diseases. Switch to ADT and gradually reduce the amount of corticosteroid administered every other day. If this is difficult, it is preferable to administer 3 or even 4 times the daily dose every other day rather than merely doubling the daily dose.

Precautions

This medication should not be used in certain patients. For example, it is contraindicated in patients who are allergic to this drug or its components, those with infections, herpes simplex, herpes zoster, varicella, or those receiving live vaccines. Additionally, patients with genetic issues such as lactose intolerance should not use this drug. Caution is advised when administering this medication to patients with or a history of various conditions such as glaucoma, tuberculosis, cataracts, hypertension, thrombosis, peptic ulcers, mental illness, osteoporosis, diabetes, and renal failure. Various side effects, including edema, hypertension, muscle weakness, osteoporosis, gastrointestinal issues, skin changes, psychiatric symptoms, endocrine disorders, and eye problems, may occur during the use of this medication, so careful monitoring is required. Long-term use may lead to adrenal insufficiency; therefore, it is essential not to stop suddenly but to taper the dose gradually, and it may be necessary to increase the dose during stressful situations. Because this medication suppresses the immune system, patients may become susceptible to infections, particularly varicella or measles, and immediate treatment is required if exposed. This medication may interact with other drugs, so those taking other medications should consult a doctor or pharmacist. In cases of pregnancy or breastfeeding, this medication may affect the fetus or newborn, so it is crucial to consult a physician before use. In children, side effects such as growth inhibition may occur, and the minimum effective dose should be used while closely monitoring growth status. Elderly patients are at higher risk for side effects such as infections, diabetes, osteoporosis, and glaucoma, hence administration should be cautious. Lastly, if exposed to varicella or measles while taking this medication, serious outcomes may occur, so vaccination status should be checked, and infection risk should be minimized.
