Treatment of gastroesophageal reflux disease (such as erosive esophagitis) and treatment of Helicobacter pylori positive peptic ulcer, and prevention of gastric ulcers associated with non-steroidal anti-inflammatory drugs.
This medication is used for the treatment of gastroesophageal reflux disease, particularly erosive esophagitis, and for long-term maintenance therapy to prevent recurrence. It also helps in relieving symptoms of gastroesophageal reflux disease without esophagitis. It is used in combination with antibiotics for eradication of Helicobacter pylori for the treatment and prevention of recurrence of duodenal ulcers and peptic ulcers. It is also effective for the treatment of upper gastrointestinal symptoms associated with non-steroidal anti-inflammatory drug use, as well as for the prevention of gastric ulcers and duodenal ulcers. This medication is also applicable for the treatment of Zollinger-Ellison syndrome and for maintenance therapy to prevent rebleeding of gastric or duodenal ulcers. Overall, it is widely used for the treatment of various symptoms and diseases related to gastrointestinal disorders.
This medication should be swallowed whole with liquids such as water and should not be chewed or crushed. 1) Gastroesophageal reflux disease (GERD) - Treatment of erosive esophagitis: 40 mg once daily for 4 weeks. - If esophagitis remains untreated or symptoms persist, continue for 4 more weeks. - Long-term maintenance therapy for prevention of recurrence in patients with esophagitis: 20 mg once daily. - For symptoms of gastroesophageal reflux disease without esophagitis: · 20 mg once daily. If symptoms are not controlled after 4 weeks, additional medical attention is needed. · Once symptoms improve, subsequent symptoms can be managed with 20 mg once daily. · In adults, 20 mg may be administered as needed once daily. Patients at high risk for gastric ulcers and duodenal ulcers from non-steroidal anti-inflammatory drug use are not recommended to receive this on a continuous basis for symptom control. 2) Antibiotic combination therapy for eradication of Helicobacter pylori This medication is administered at 20 mg with amoxicillin 1 g and clarithromycin 500 mg, given twice daily for 7 days. 3) Short-term therapy for upper gastrointestinal symptoms (pain, discomfort, burning) associated with non-steroidal anti-inflammatory drug (COX-2 non-selective and selective) administration - 20 mg once daily. If symptoms are not controlled after 4 weeks, additional medical attention is needed. Clinical trials extending beyond 4 weeks have not been conducted. 4) For patients requiring continuous non-steroidal anti-inflammatory drug administration - Treatment of gastric ulcers associated with non-steroidal anti-inflammatory drug administration: 20 mg once daily. The treatment duration is 4 to 8 weeks. - Prevention of gastric and duodenal ulcers associated with non-steroidal anti-inflammatory drug administration: 20 mg once daily. 5) Treatment of Zollinger-Ellison syndrome The recommended starting dose is 40 mg twice daily. Subsequent doses should be adjusted based on the patient's condition, and treatment should continue as long as clinical symptoms persist. Clinical data suggest that most patients can be managed on doses ranging from 80 to 160 mg daily. Doses exceeding 80 mg daily should be divided into two administrations. 6) Maintenance therapy after intravenous induction for prevention of rebleeding due to gastric or duodenal ulcer After intravenous induction for rebleeding prevention due to gastric or duodenal ulcers, administer 40 mg once daily for 4 weeks. 2. Adolescents over 12 years 1) Gastroesophageal reflux disease (GERD) - Treatment of erosive esophagitis: 40 mg once daily for 4 weeks. If esophagitis remains untreated or symptoms persist, continue for 4 more weeks. - Symptomatic treatment for gastroesophageal reflux disease without esophagitis: · 20 mg once daily. If symptoms are not controlled after 4 weeks, additional medical attention is needed. · Once symptoms improve, subsequent symptoms can be managed with 20 mg once daily. · In adults, 20 mg may be administered as needed once daily. Patients at high risk for gastric ulcers and duodenal ulcers from non-steroidal anti-inflammatory drug use are not recommended to receive this on a continuous basis for symptom control. 3. Children under 12 years: There is no experience with administering this medication to children under 12. 4. Renal impairment: No need for dosage adjustment. There are not many examples of administration in patients with severe renal failure, so careful consideration is needed. 5. Hepatic impairment: No need for dosage adjustment in patients with mild to moderate liver impairment. In patients with severe liver impairment, the dosage should not exceed 20 mg. 6. Elderly: No need for dosage adjustment.

This medication should not be used in certain patients. It should not be taken by individuals who are allergic to this drug or its ingredients, or have a history of hypersensitivity to benzimidazole-class medications. Additionally, patients who are allergic to penicillin antibiotics or macrolide antibiotics should not use this medication in combination. In particular, there is a risk of side effects when used in conjunction with certain heart-related medications or antivirals, so it should be avoided. It is also safer for breastfeeding women not to take this medication. This medication can cause various adverse reactions. For example, it may result in decreased blood cell counts, abnormal immune responses, electrolyte imbalances, neurological symptoms, digestive issues, etc. Particularly, hypomagnesemia can cause serious side effects and requires attention. Reports of skin rashes, severe allergic reactions, liver dysfunction, muscle issues have also been noted. If any of these symptoms occur, consult a healthcare provider immediately. Long-term use of this medication can mask malignancy symptoms or delay diagnosis, so consult a specialist if suspicious symptoms arise. Regular check-ups to monitor health status are important for those who take this medication for more than one year. Furthermore, this medication may interact with other drugs, so always consult a doctor when symptoms change or when taking other medications. This medication may reduce gastric acid secretion and lead to an increase in the number of bacteria in the gastrointestinal tract, which may slightly increase the risk of infections. Hospitalized patients or those with weakened immunity need to exercise particular caution, and if diarrhea persists, they should visit a hospital immediately. Long-term use may increase the risk of fractures, so patients at risk of osteoporosis should receive proper management, including bone density testing per the doctor's instructions. Additionally, this medication contains sugars, so patients with certain metabolic disorders of sugars should not take it. This medication can interact with multiple drugs, so if taking other medications, always inform healthcare professionals. Particularly when used with anticoagulants, anticonvulsants, antidepressants, or antivirals, blood concentration changes and side effect risks should be monitored. The data on the safety of this medication during pregnancy is limited, so pregnant women should consult a doctor before taking it. Breastfeeding women are also advised not to take this medication. There are no specific antidotes in case of overdose, so treatment should be symptomatic and general supportive care is necessary. Keep this medication out of reach of children and do not transfer it to another container.
