This medication is used for preparing dialysis solutions for patients with acute and chronic renal failure, and it is also used for dialysis in cases of detoxification from toxic substances. Therefore, it is used to assist in dialysis treatment for patients with decreased kidney function or those exposed to toxic substances.

Dosage and Administration

Adult dosage

When using the defined acetate-containing dialysis solution (Solution A), this preparation can create a sodium bicarbonate-based dialysis solution containing HCO3- at 35 mEq/L. It is used as dialysis fluid in hemodialysis when using a dilution ratio of 1:1.225:32.775 of acetate-containing dialysis solution (Solution A), this preparation, and purified water.

Precautions

This medication should not be used in patients with low blood pressure and rapid heart rate. It should also be avoided in patients with pulmonary edema or severe changes in electrocardiogram. Patients with high serum potassium levels and those with blood clotting issues should not be administered this medication. Caution should be exercised when using this medication in patients with severe liver impairment or those who have metabolic issues due to diabetes. Patients taking digitalis preparations should be careful as this medication may decrease serum potassium levels, increasing the risk of drug toxicity. Patients with high serum calcium and phosphorus levels before dialysis should also use this medication with caution. Since serum calcium levels may increase, dialysis solutions should be adjusted if hypercalcemia is suspected. Osteoporosis or osteomalacia-related complications may occur, so appropriate treatment such as vitamin D3 supplementation is necessary. Abnormal calcium deposition may happen, so maintaining serum phosphorus levels within the normal range is crucial. During dialysis, symptoms such as headache, nausea, vomiting, convulsions, and altered consciousness may present, hence dialysis intensity should be adjusted in these cases. It can influence the circulatory system, potentially causing symptoms of hypotension or shock, which necessitates stopping or adjusting dialysis and receiving appropriate treatment if symptoms appear. This medication is used in dialysis for patients with chronic renal failure and is suitable when it is difficult to improve metabolic acidosis, or when hypotension or dialysis disequilibrium syndrome occurs. It is also used for patients who find it difficult to control hyperglycemia due to glucose metabolism abnormalities. During use, it is advisable to regularly check electrolytes, acid-base balance, and blood sugar via blood tests. Pregnant women or those who may become pregnant should only use this medication if the therapeutic benefits outweigh the risks, as its safety has not been established.
