Treatment of hypercholesterolemia and dyslipidemia (primary, familial) and reduction of cardiovascular disease risk
This medicine is primarily used when dietary management and exercise alone are insufficient for controlling primary hypercholesterolemia and combined hyperlipidemia in patients. It is especially used as an adjunct to lipid control in patients with familial hypercholesterolemia, which helps lower total cholesterol and LDL cholesterol levels, thereby slowing the progression of atherosclerosis. Additionally, it is used in certain pediatric patients to control high cholesterol levels that remain despite dietary adjustments. It is also used alongside dietary management in patients with dysbetalipoproteinemia (DBL). In patients at high risk for cardiovascular disease, it is effective in reducing the risk of stroke, myocardial infarction, and arterial revascularization procedures. Therefore, this drug is used for hyperlipidemia and reducing the risk of cardiovascular disease.
For adults with primary hypercholesterolemia, the medication can be taken at any time of the day, regardless of meals. The initial dosage is 5 mg once daily, which may be adjusted as a maintenance dose if necessary. A typical maintenance dose is 10 mg once daily, which should be adjusted based on LDL cholesterol levels, treatment goals, and patient response with intervals of at least 4 weeks. The maximum dose is 20 mg once daily.
For pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17 years), the recommended dosage is 5 mg to 20 mg once daily. The dosage should be adjusted individually based on the treatment goals and evaluated after a 4-week interval.
This medicine should not be administered to certain patients. For example, individuals allergic to any component of this medicine or those with severe liver dysfunction should not use it. Patients with muscle disorders or those taking specific immunosuppressants should also avoid this medication. Women who are pregnant or breastfeeding, as well as fertile women who do not practice appropriate contraception, should avoid using this medicine. This medication contains lactose, so it is not suitable for patients with lactose intolerance. Regular monitoring of liver function and muscle health is required while using this drug. Caution is especially needed in individuals who consume excessive alcohol or have liver disease. If symptoms such as muscle pain or weakness occur, medical personnel should be informed immediately. Older adults, as well as individuals with impaired renal function or hypothyroidism, should use this medication cautiously. Because this drug may interact with other medications, it is essential to inform healthcare providers of any other drugs being taken. The risk of muscle abnormalities may increase when taken with certain antibiotics, antifungals, immunosuppressants, or fibrate class medications. This drug should not be used during pregnancy, and women planning to become pregnant should consult with a healthcare provider before use. It is also advisable to avoid this medication while breastfeeding, as it may be transmitted through breast milk. In case of overdose, there is no specific antidote, so one should seek appropriate medical treatment immediately. Caution is advised when driving or operating machinery, as dizziness may occur after taking this medication. Regular check-ups to monitor liver and muscle health during the treatment period are crucial.
