Treatment of acute and chronic renal failure (peritoneal dialysis), detoxification of drug poisoning
This medication is used as a peritoneal dialysis solution for patients with acute and chronic renal failure, and is also utilized as an antidote in cases of drug poisoning where other treatments are ineffective. It also helps alleviate discomfort caused by a low pH that may occur when using lactate-based dialysis solutions. Therefore, it is used for peritoneal dialysis in renal failure patients, detoxification from drug poisoning, and improving discomfort related to dialysis solutions.
In adults, 1.5 to 2.5L is injected into the abdominal cavity once. In continuous ambulatory peritoneal dialysis, the drainage process is performed 3 to 5 times a day after 4 to 8 hours of retention. In automated peritoneal dialysis, 4 to 5 cycles are performed overnight using a machine, and up to 2 times during the day.
This medication is for use in peritoneal dialysis, and discontinuing treatment early may pose a threat to life, so it must be used according to a doctor's instructions. It should not be used in patients with a perforated diaphragm, severe abdominal burns, severe peritoneal adhesion, hemorrhagic disorders, lactate metabolism disorders, or severe hypokalemia or hypocalcemia. Patients who have had recent abdominal surgery or injury, inflammatory bowel disease, abdominal fistula, intra-abdominal tumors, abdominal hernia, bowel obstruction, severe respiratory disease, sepsis, severe lipid metabolism disorders, cachexia, or significant weight loss should also avoid administration. Additionally, it is not recommended for patients who are physically or mentally unable to comply with treatment instructions. Caution should be taken when using this medication in patients taking digitalis preparations, those with glucose metabolism disorders, lumbar disorders, colostomy, diuretics users, patients with severe ventilation disorders, severe obesity, steroid users, immunocompromised patients, pregnant women, patients with peritonitis, or patients with primary hyperparathyroidism, requiring regular blood tests and monitoring. While using this medication, one must carefully monitor for possible adverse reactions such as abdominal pain, bleeding, peritonitis, inflammation around the catheter, and more. Additionally, various side effects may occur, including nausea, diarrhea, constipation, electrolyte imbalance, metabolic disorders, respiratory distress, edema, fever, muscle cramps, and blood pressure changes. Due to the risk of infection during peritoneal dialysis, it should be administered in a clean and sterile environment, and the clarity and drainage status of the dialysis solution should be continuously checked. Long-term use may lead to decreased peritoneal function, so regular checks of the patient's condition are necessary, and treatment methods should be changed as needed. This medication requires careful management of fluid balance and nutritional status, and diabetic patients should monitor their blood sugar levels frequently and adjust their insulin dosage accordingly. In case of overdose, dehydration, electrolyte abnormalities, and hyperglycemia may occur, and medical attention must be sought immediately, and the dialysis solution container should only be used if it is not damaged. Peritoneal dialysis solutions should only be used for peritoneal dialysis, and they are best used when warmed to body temperature. In children and the elderly, adjustments to dosing should be made, especially considering the risk of hernia in the elderly. The safety of this medication in pregnant or nursing women has not been established; it should only be used when the need for treatment outweighs the risks. Finally, this medication does not affect the ability to drive or operate machinery.
