Relief of severe and moderate acute and chronic pain (e.g. pain from cancer, diagnoses, and postoperative pain)
This medication is used to relieve severe and moderate acute and chronic pain, for example, pain associated with various cancers or pain that occurs after diagnosis and surgery.
As tramadol hydrochloride, administer 50-100 mg intravenously or intramuscularly as needed, repeating every 4-5 hours. The maximum daily dose can be up to 400 mg.
Exceeding the recommended dosage may cause seizures. Caution should be exercised in patients with a history of epilepsy or those at risk of seizures, and it should only be used when necessary. Prolonged use may lead to psychological and physical dependence, thus it should be administered for a short period under strict supervision in patients with a history of substance abuse or dependence. Combination with alcohol or central nervous system depressants (such as benzodiazepines) can result in severe sedation, respiratory depression, coma, and even death, so it should be avoided or used with extreme caution. Patients with certain genetic variations may metabolize this medication faster, leading to increased blood levels and a higher risk of respiratory depression, therefore dosage adjustment or alternative medication should be considered for these patients. This medication should not be used in children under 12 years or in adolescents under 18 years who have undergone tonsillectomy or adenoidectomy. Should not be administered in cases of acute alcohol intoxication, central nervous system drug intoxication, severe respiratory depression, head injury, patients currently taking MAO inhibitors, patients with severe renal or hepatic impairment, and pregnant or breastfeeding women. Patients taking morphine, those with liver or kidney diseases, alcohol users, and those with a history of seizures must use this medication cautiously, and patients with a history of substance abuse or mental illness should also be monitored closely. This medication may cause side effects such as drowsiness, dizziness, nausea, and vomiting; therefore, activities such as driving or operating dangerous machinery should be avoided after administration. Prolonged use may lead to tolerance, requiring increased doses, and sudden discontinuation can provoke withdrawal symptoms; therefore, gradual dosage reduction should be implemented. This medication may interact with other central nervous system depressants, antidepressants, antipsychotics, etc., increasing the risk of side effects, so consultation with a physician is essential. It is advisable for pregnant or breastfeeding women to avoid this medication, particularly during lactation, as it may cause serious side effects in the baby. Overdosing may result in severe respiratory depression and seizures; immediate emergency treatment is required, and naloxone may be used as an antidote. It is safest not to use this medication in children, especially those under 12 years and in specific postoperative adolescents under 18 years. Elderly patients (75 years and older) may experience increased blood levels, thus the maximum daily dose should be limited. This medication must be stored out of reach of children and should be kept in the original container for safety.
