This medication is used in combination with diet and exercise to improve blood sugar control in patients with type 2 diabetes. It may be administered alone when starting therapy, and if blood sugar control is insufficient with monotherapy, it can be used in combination with metformin. Additionally, it may be used in conjunction with sulfonylureas, thiazolidinediones, or insulin when blood sugar control is inadequate with monotherapy. Moreover, for those who are insufficiently controlled on combinations of sulfonylureas with metformin, thiazolidinediones with metformin, or insulin with metformin, this drug can be added to enhance treatment. In this way, this medication effectively improves blood sugar control at various stages of treatment.

How to take

Adult dose

The recommended dose is 100 mg once daily for monotherapy or combination therapy, with a maximum daily dose of 100 mg. This medication can be taken regardless of meals. Dosage adjustment is necessary depending on renal function. - For patients with mild to moderate renal impairment (eGFR ≥ 45 and < 60), no dosage adjustment is needed. - For patients with moderate renal impairment (eGFR ≥ 30 and < 45), the dose should be 50 mg once daily.

Precautions

This medication may cause serious allergic reactions in some patients; therefore, if symptoms such as a skin rash or difficulty breathing appear within 3 months of administration, it should be discontinued immediately and a physician should be consulted. This medication can induce severe pancreatic disease known as acute pancreatitis, so symptoms such as abdominal pain or vomiting should be reported to a physician immediately. It is not for use in patients with type 1 diabetes, diabetic ketoacidosis, or those with hypersensitivity to this drug or its components. Patients with impaired renal function may experience delayed excretion of this drug; thus, dosage adjustments should be made according to a physician's directions. This medication poses a risk of hypoglycemia when taken alone or with other diabetes medications, so blood glucose should be monitored carefully, especially when used with insulin or sulfonylureas. Severe joint pain or conditions that cause blisters on the skin may occur; if such symptoms arise, the medication should be discontinued and a physician should be consulted immediately. Women who are pregnant or breastfeeding should avoid using this medication, and safety in children and adolescents under 18 years has not yet been established. Elderly patients are at a higher risk of renal function decline, so regular monitoring of renal function should be performed before and during administration. In case of overdose, measures should be taken to remove the drug from the gastrointestinal tract and clinical monitoring such as electrocardiogram should be conducted, and if necessary, certain drugs can be removed through dialysis. This medication has minimal significant interactions with other drugs; however, if taking certain medications, informing a physician and appropriate monitoring is required. Patients should report any unusual symptoms or side effects to healthcare professionals immediately.
