Reduction of the risk of myocardial infarction and stroke and improvement of hyperlipidemia
This medication helps reduce the risk of cardiovascular disease. It particularly decreases the risk of myocardial infarction and stroke in the elderly, smokers, those with hypertension, and those with a family history. It also aids in reducing the risk of cardiovascular disease in diabetic patients. For patients who already have heart disease, it reduces the risk of hospitalization due to myocardial infarction, stroke, angina, and congestive heart failure. Additionally, when used alongside dietary therapy for hyperlipidemia, it is useful in lowering total cholesterol, LDL cholesterol, and triglyceride levels, and in raising HDL cholesterol levels to help maintain healthy blood lipid levels. For pediatric patients aged 10 to 17 years with familial hypercholesterolemia, it is used as an adjunct to diet to effectively control blood cholesterol levels. Overall, this medication is used to improve cardiovascular disease and hyperlipidemia.
The recommended initial dose for hyperlipidemia is 10 mg of atorvastatin once daily, or if greater LDL cholesterol reduction is required, start at 20 mg or 40 mg once daily. This medication is administered in a dosage range of 10 to 80 mg once daily. The initial and maintenance doses should be individualized based on the treatment goal and patient response, and lipid levels should be analyzed within 2 to 4 weeks after initiation and/or dose adjustment to adjust the dosage accordingly. The treatment aim is to lower LDL cholesterol levels; therefore, LDL cholesterol levels should be used for both the start of treatment and evaluation of treatment response. If LDL cholesterol levels cannot be used, total cholesterol levels should be used to monitor treatment response. For patients with homozygous familial hypercholesterolemia, the dosing regimen is 10 to 80 mg once daily, used either in conjunction with other lipid-lowering treatments (e.g., LDL apheresis) or when such treatments are not possible. In pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17), the recommended initial dose is 10 mg once daily, and the recommended maximum dose is 20 mg once daily (there is no clinical data for doses higher than 20 mg in this age group). The dosage should be adjusted appropriately based on the recommended therapeutic target for each individual patient. Dosage adjustments should be made with an interval of 4 weeks or more. ○ Patients with renal impairment: Renal disease does not affect atorvastatin plasma concentration or LDL cholesterol-lowering effect; therefore, dose adjustment is unnecessary. ○ Elderly patients: Efficacy and safety in patients over 70 years old are similar to that of average adult patients.
The recommended initial dose for pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17) is 10 mg once daily, with a maximum dose of 20 mg once daily. Dosage adjustments should be made with an interval of 4 weeks or more.

Elderly patients are at a higher risk of muscle-related side effects, so they require close monitoring while taking this medication.
