Treatment and prevention of gastroesophageal reflux, gastroduodenal disorders (such as ulcers), and Zollinger-Ellison syndrome
This medication is used for the short-term treatment of gastric and duodenal ulcers, alleviating symptoms such as heartburn and regurgitation associated with gastroesophageal reflux disease (GERD). It is also helpful in treating Zollinger-Ellison syndrome and is used for maintenance therapy for severe gastroesophageal reflux disease and peptic ulcers. It is used in combination with antibiotics to prevent the recurrence of duodenal ulcers infected with Helicobacter pylori, and it is effective in the treatment and prevention of gastric and duodenal ulcers or erosions caused by NSAID use. It is particularly useful in preventing gastric and duodenal ulcers and dyspeptic symptoms in patients on long-term NSAIDs and in preventing the recurrence of gastroesophageal reflux disease.
Ⅰ) Adults 1) Gastric ulcer: Administer omeprazole 20mg per dose orally once daily. Most ulcers may heal within 4 weeks, although the healing period may vary among patients. If healing is not complete after the initial 4 weeks of treatment, continue treatment for an additional 4 weeks. In patients who have not responded to other medicines, administration of 40mg per dose once daily generally results in healing within 8 weeks, although the healing period may vary among patients. Repeat treatment if the ulcer recurs. 2) Short-term treatment of duodenal ulcer: Administer 20mg per dose orally once daily. Most ulcers may heal within 2 weeks, although the healing period may vary among patients. If healing is not complete after the initial 2 weeks of treatment, continue treatment for an additional 2 weeks. In patients who have not responded to other medicines, administration of 40mg per dose once daily generally results in healing within 4 weeks, although the healing period may vary among patients. Repeat treatment if the ulcer recurs. 3) Reflux esophagitis: Administer 20mg per dose orally once daily. Most cases may heal within 4 weeks, although the healing period may vary among patients. If healing is not complete after the initial 4 weeks of treatment, continue treatment for an additional 4 weeks. In patients who have not responded to other medicines, administration of 40mg per dose once daily generally results in healing within 8 weeks, although the healing period may vary among patients. Repeat treatment if the condition recurs. 4) Zollinger-Ellison syndrome: The initial dose is 60mg per dose orally once daily. Continue treatment, adjusting the dose appropriately for each individual. More than 90% of patients respond to doses of 20 - 120mg per day. If more than 80mg per day is administered, divide the dose into 2 doses per day. 5) Maintenance therapy for severe reflux esophagitis and peptic ulcers that are difficult to treat: Administer 20mg per dose orally once daily. If the condition recurs, the dose may be increased to 40mg per dose once daily. 6) Combination therapy with antibiotics to prevent recurrence of duodenal ulcers infected with Helicobacter pylori: - Dual therapy: Administer this medicine 20mg with amoxicillin 750 - 1000mg twice daily for 2 weeks. - Triple therapy: Administer this medicine 40mg once daily or 20mg twice daily in combination with the following antibiotics. ① Administer clarithromycin 250mg and metronidazole 400mg twice daily for 1 week, or ② Administer amoxicillin 1g and clarithromycin 500mg twice daily for 1 week. If the therapeutic effect is insufficient, omeprazole monotherapy may be continued. 7) Treatment of symptoms of gastroesophageal reflux disease (GERD) (heartburn, regurgitation, etc.): Administer 20mg per dose orally once daily. Treatment is generally possible with 2-4 weeks of use. 8) Treatment of gastric and duodenal ulcers or erosions caused by nonsteroidal anti-inflammatory drugs (NSAIDs): Administer 20mg per dose orally once daily. Most cases may heal within 4 weeks, although the healing period may vary among patients. If healing is not complete after the initial 4 weeks of treatment, continue treatment for an additional 4 weeks. 9) Maintenance prevention of gastric and duodenal ulcers or erosions and dyspeptic symptoms caused by NSAID use in patients with a history of gastric or duodenal lesions who are receiving long-term NSAID treatment: Administer 20mg per dose orally once daily. * Swallow this medicine whole with at least half a glass of water. Adjust the dose as appropriate according to age and symptoms.
This medication should not be used in patients allergic to its components. In patients with gastric ulcers, if there is a possibility of malignant tumors, symptom relief can delay diagnosis, requiring caution. Additionally, proton pump inhibitors like this should not be used in conjunction with certain antiviral agents, so combinations should be avoided. This medication should not be given to patients with congenital lactose metabolism disorders due to the lactose contained in it. Dosing should be adjusted or used cautiously in patients with impaired liver function or the elderly. Symptoms such as headache, drowsiness, dizziness, and rarely psychotic symptoms may occur. Since liver function can be affected, if there are abnormal liver enzyme levels, the use of this drug should be stopped. Common digestive system symptoms include diarrhea, nausea, abdominal pain, as well as skin symptoms such as rash or itching. Severe allergic reactions such as angioedema, fever, and anaphylactic shock can occur rarely as hypersensitivity reactions. Various side effects, including decreased blood cell counts, myalgia, gynecomastia, and visual disturbances, have been reported, so consultation with a physician is necessary if symptoms arise. Long-term use may increase the risk of fractures and hypomagnesemia, requiring regular check-ups. This medication may affect the absorption of other drugs by suppressing gastric acid secretion, so interaction with concomitant medications should be monitored. Women who are pregnant or may become pregnant should use this medication cautiously, and nursing mothers should either avoid administration or discontinue breastfeeding. The safety for children has not been sufficiently established, so caution is needed in their use. In case of overdose, symptoms such as vomiting, diarrhea, and headache may appear, but most are temporary, and severe outcomes have not been reported.
