How to take

Adult dose

This medication is taken orally once daily at the same time, regardless of meals. For hypertension treatment, the initial and maintenance dose is 50 mg once daily. If the therapeutic effect is insufficient, the dose may be split into two doses per day or increased to a maximum of 100 mg once daily. For treating kidney disease in hypertensive patients with type 2 diabetes, 50 mg is generally administered once daily, with adjustments based on blood pressure, up to 100 mg once daily.

Precautions

This medication should not be taken by pregnant women, especially during the second and third trimesters, as it can cause serious harm to the fetus and newborn. It should not be administered to patients with allergies to this medication, severely impaired liver function, history of angioedema, primary hyperaldosteronism, and those with certain genetic lactose metabolism disorders. Elderly patients, those with slightly compromised kidney function, patients with hyperkalemia or those prone to elevated serum potassium levels, and patients with heart valve disease or cardiac hypertrophy should use this medication with caution, ensuring that blood pressure does not drop too low. While taking this medication, side effects such as muscle cramps, dizziness, cough, and upper respiratory tract infections may occur. Rarely, serious adverse reactions such as angioedema, liver dysfunction, renal failure, cardiac arrhythmias, and hypoglycemia may present, requiring immediate consultation with a healthcare professional if symptoms arise. Patients with compromised liver or kidney function may experience elevated blood concentrations or worsening kidney function while taking this medication, necessitating dose adjustments or regular monitoring. Severe renal failure patients must be particularly careful. When taken in conjunction with potassium-sparing diuretics, potassium supplements, or potassium-containing salt substitutes, this medication may cause dangerously elevated blood potassium levels; hence, these combinations should be avoided. Caution should be exercised when used with non-steroidal anti-inflammatory drugs (NSAIDs), as it may diminish the hypotensive effect and increase the risk of renal impairment. When combined with lithium, there is a risk of lithium toxicity; regular monitoring of serum levels is essential. Use of this medication during pregnancy increases the risks of reduced fetal kidney function, decreased amniotic fluid, skeletal deformities, and hypotension; therefore, it should be discontinued immediately upon confirmation of pregnancy. Lactating women should also avoid taking this medication. Regular monitoring of blood pressure, kidney function, and electrolyte levels is essential during treatment, and if dizziness or hypotension occurs, driving or operating machinery should be avoided.
