How to take

Adult dose

The dosage of this medication should be determined taking into account each patient's current treatment regimen, efficacy, and tolerability, not exceeding sitagliptin 100 mg and metformin 2000 mg. Generally, it is administered twice a day with food, gradually increasing the dose to reduce gastrointestinal adverse reactions caused by metformin. 1. For patients not currently treated with metformin: The initial dose is 1 tablet (50 mg) twice daily.

Precautions

This medication contains both sitagliptin and metformin. First, serious allergic reactions may occur during the use of this medication, so if symptoms such as rash, hives, difficulty breathing, or swelling of the face or neck occur, the medication should be stopped immediately and medical attention should be sought. Additionally, there is a risk of pancreatitis, which is an inflammation of the pancreas; therefore, sudden abdominal pain, nausea, or vomiting should be watched carefully, and if these symptoms occur, immediate consultation with a physician is required. Due to the metformin component, lactic acidosis, which is a rare but potentially life-threatening condition, may occur; symptoms such as muscle pain, shortness of breath, fatigue, or abdominal pain should lead to an immediate cessation of this medication and notification to healthcare personnel. This medication should not be taken by patients with severely impaired kidney function, severe infections or dehydration, heart failure, or conditions affecting kidney function such as acute myocardial infarction. Furthermore, if undergoing tests that involve using iodinated contrast agents, this medication should be discontinued for a period of time before and after the test, and should only be resumed once kidney function has recovered to normal. Women who are pregnant or may become pregnant, as well as nursing mothers, should not take this medication. During use of this medication, irregular or insufficient food intake, intense exercise, or use of additional medications may increase the risk of hypoglycemia or lactic acidosis, so caution is advised. During treatment, side effects such as diarrhea, nausea, vomiting, abdominal pain, headaches, and upper respiratory infections may occur, and symptoms of hypoglycemia may also arise, so it is crucial to monitor blood sugar levels. In particular, when taking concurrently with insulin or sulfonylureas, there is an increased risk of hypoglycemia, thus dosage adjustments may be necessary according to a doctor's directive. Regular blood tests and kidney function tests should be conducted, and supplementation may be needed for potential Vitamin B12 deficiency. Excessive alcohol intake may increase the risk of side effects from this medication, so it is advisable to avoid it. In case of surgery, consultation should be initiated 48 hours prior to the procedure to stop taking this medication, and it should be resumed only after recovery of oral intake and confirmation of normal kidney function. If sudden joint pain or skin blistering occurs during treatment, immediate notification to a physician is required, and discontinuation of medication may be necessary if deemed appropriate. Finally, this medication is not recommended for use in children under 18 years of age, as safety and efficacy have not been established.

Serious side effects

Hives, severe swelling (angioedema), chest tightness, difficulty breathing, anaphylaxis, Stevens-Johnson syndrome (exfoliative skin disease), severe rash with high fever, blistering, peeling of skin, mucosal pain in mouth/eyes, elevated liver enzymes, liver abnormalities (jaundice), acute pancreatitis (hemorrhagic or necrotizing pancreatitis, fatal or non-fatal), acute renal failure, hypoglycemia (severe cases include confusion and seizures), severe swelling, high blood pressure.

Mild side effects

Diarrhea, nausea, vomiting, abdominal pain, headache, upper respiratory infections, nasopharyngitis, constipation, muscle pain, limb pain, back pain, itching, thrombocytopenia, rhabdomyolysis, dizziness, decreased appetite, weakness, indigestion, Vitamin B12 deficiency, peripheral edema, severe joint pain accompanied by disability, blistering dermatitis.
