Efficacy & effects

Improvement of blood sugar control in type 2 diabetes

  • Blood sugar control issues in patients with type 2 diabetes
View approved original text

This medication is used to improve blood sugar control in patients with type 2 diabetes. It serves as an adjunct to dietary and exercise therapy, and can be administered as monotherapy. However, if blood sugar control is insufficient, it may be used in combination with oral hypoglycemic agents or insulin. Particularly, when control is difficult with sulfonylureas or metformin monotherapy, it may be co-administered with metformin to enhance blood sugar control. These various administration methods effectively assist patients in managing their blood sugar levels.

How to take

Adult dose

Individualized for each patient. Swallowed whole without chewing with more than 1/2 cup of water. Administered orally once daily. It should be taken before breakfast or immediately before the first meal, and it is very important not to skip meals after taking the medication. The initial dose for monotherapy is 1 mg of glimepiride once daily, which can be increased by 1 mg at 1-2 week intervals. Generally, when administered above 4 mg, additional effects are minimal.

Precautions

This medication should not be used in patients with insulin-dependent diabetes, diabetic ketoacidosis, or in a comatose state. Additionally, it should not be administered to patients who have had hypersensitivity reactions to this medication or to sulfonylureas or sulfonamide medications. Experience with using this medication in patients with severe liver or kidney impairment is lacking, and in such cases, switching to insulin therapy is recommended. Women who are pregnant or may become pregnant and women who are breastfeeding should not take this medication. Patients with severe infections, undergoing surgery, or with major trauma should use insulin therapy, and in cases of gastrointestinal disorders like diarrhea or vomiting, absorption of the medication may decrease, increasing the risk of hypoglycemia. This medication contains lactose, so patients with lactose intolerance or certain inherited metabolic disorders should not take it. During the first week of dosing, the risk of hypoglycemia is high, so careful monitoring of blood sugar is especially necessary. High-risk patients for hypoglycemia include the elderly, those with poor nutritional status or irregular eating patterns, and those with excessive exercise and dietary imbalances. Additionally, patients who frequently consume alcohol or have impaired kidney and liver function, as well as those with endocrine disorders, require caution. Co-administration with other medications may influence blood sugar control, so patients should inform their doctor or pharmacist about any medications they are taking. This medication may cause hypoglycemia, with symptoms including headache, severe hunger, nausea, fatigue, anxiety, confusion, drowsiness, and loss of consciousness. Severe hypoglycemia may present with symptoms similar to a stroke, requiring immediate treatment. Some patients may experience visual disturbances, nausea, vomiting, abdominal pain, liver function abnormalities, blood-related reactions, and allergic reactions. If severe allergic reactions such as hives or difficulty breathing occur, medical attention should be sought immediately. This medication should be taken regularly, along with dietary and exercise measures, and it is important to maintain good blood sugar control. If hypoglycemia occurs, it can usually be promptly controlled by sugar intake, so patients should always carry sugar. During treatment, regular blood sugar and urine sugar level tests should be conducted, and treatment methods adjusted as needed. Additionally, caution is advised when driving or operating machinery due to impaired agility from hypoglycemia or hyperglycemia. This medication can interact with multiple drugs, so patients should inform healthcare personnel about other medications they are taking. In particular, changes in blood sugar may occur when taken with certain antibiotics, anticoagulants, steroids, or beta-blockers. Women who are pregnant or breastfeeding may pose risks to the fetus or neonate; therefore, insulin therapy is recommended instead of this medication. Children under 18 years of age should not use this medication as its safety and efficacy have not been established. Elderly patients may have reduced kidney function, so dosage adjustments should be made with caution. In cases of overdose, severe hypoglycemia may occur, requiring emergency measures such as inducing vomiting, gastric lavage, or intravenous glucose injection. This medication must be kept out of reach of children and stored in its original container to prevent misuse and deterioration of quality.
