This medication is used to improve atherosclerotic symptoms in adult patients with ischemic stroke, myocardial infarction, and peripheral arterial disease. Additionally, it helps reduce the risk of cardiovascular-related complications in patients with acute coronary syndrome, unstable angina, or non-Q wave myocardial infarction. It is also applicable to patients who have undergone coronary intervention or coronary artery bypass, aiding in the improvement of myocardial infarction, stroke, and other ischemic symptoms. Furthermore, in patients with atrial fibrillation who have vascular risk factors and where the use of vitamin K antagonists is unsuitable, if the bleeding risk is low, it is effective in reducing the risk of stroke and thromboembolism. In summary, this medication is used to effectively improve and prevent various vascular disease-related symptoms and risks.

Dosage and Administration

Adult dosage

1. For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, 75 mg of Clopidogrel is administered orally once daily. 2. For patients with acute coronary syndrome, start with a loading dose of 300 mg orally once daily on the initiation day, followed by a maintenance dose of 75 mg orally once daily. Precautions

This medication prevents platelets from aggregating, facilitating good blood flow. The effectiveness of this medication may be diminished in individuals with genetically reduced function of a specific enzyme, CYP2C19, leading to an increased risk of recurrent cardiac or cerebrovascular issues. Alternative treatments should be considered in such cases. Moreover, if taken alongside drugs that enhance CYP2C19 enzyme activity, the effectiveness of this medication may become excessively strong, increasing the risk of bleeding; thus, concomitant use is not recommended. Patients with allergies to this medication or its ingredients, those already experiencing bleeding, patients with severe liver damage, lactating women, and those with genetic issues such as lactose intolerance should not take this medication. Patients at high risk of bleeding, those with impaired liver or kidney function, recent stroke patients, hypertensive patients, the elderly, and underweight patients should use this medication cautiously, and especially in cases of upcoming surgery, the medication should be discontinued 5 to 7 days prior to the procedure to reduce bleeding risk. While taking this medication, if bleeding symptoms or blood abnormalities appear, the healthcare provider should be notified immediately, and blood tests may be necessary. Caution is needed when taking this medication alongside aspirin and anticoagulants (e.g., warfarin, heparin, selective serotonin reuptake inhibitors), as this may increase the risk of bleeding. During pregnancy, the risk of bleeding during delivery may increase, so it is advisable to discontinue the medication 5 to 7 days before the expected delivery. If breastfeeding, it should be considered to stop breastfeeding or to discontinue the medication due to the potential for excretion into breast milk. The safety and efficacy of this medication have not been established in children, and the elderly may have a higher risk of bleeding due to impaired kidney and liver functions, requiring caution during use. Overdose may lead to severe bleeding; there is no antidote for this medication, but platelet transfusions may control bleeding if necessary. This medication should be kept out of the reach of children and should not be transferred to other containers for safety.
