Precautions

This medication should not be used in patients with urinary retention, severe gastrointestinal diseases, myasthenia gravis, or angle-closure glaucoma. Additionally, it should not be administered to patients undergoing hemodialysis, with severe liver impairment, or severe kidney impairment, and to those with specific genetic metabolic disorders. Patients at risk for urinary retention or gastrointestinal obstruction, and those with severe renal impairment or moderate liver impairment should use it cautiously. Dose adjustment may be necessary when used in combination with strong CYP3A4 inhibitors. Caution should also be exercised in patients with anticholinergic drug use. This medication can commonly cause side effects such as dry mouth, constipation, blurred vision, urinary retention, and dry eyes, particularly at high doses, significant gastrointestinal adverse reactions have been reported. Serious allergic reactions such as angioedema or anaphylaxis may occur; if symptoms arise, administration should be discontinued immediately and appropriate treatment should be sought. Caution is necessary when driving or operating machinery due to drowsiness or blurred vision. When used with other anticholinergic drugs, effects or side effects may be enhanced and the effectiveness of certain gastrointestinal motility agents may be reduced. As this drug is primarily metabolized by the CYP3A4 enzyme, blood concentration may increase when used in combination with strong CYP3A4 inhibitors, necessitating dose adjustment. The safety of use during pregnancy has not been established, and it is recommended to avoid use while breastfeeding. Safety and efficacy in pediatric patients have also not been established, so it should not be administered to children. In case of overdose, gastric lavage or administration of activated charcoal may be necessary, and appropriate emergency treatment may be required based on the severity of symptoms. Particular caution is needed in patients with heart disease or at risk for QT prolongation.
