Treatment of inflammatory, allergic, endocrine, hematologic, neoplastic, edematous, neurologic, and infectious diseases
This medication is used for various endocrine disorders, rheumatic diseases, collagen diseases, skin diseases, allergic diseases, ophthalmic diseases, gastrointestinal diseases, respiratory diseases, blood diseases, malignant tumors, edematous diseases, neurological diseases, and other infectious diseases. For example, it is utilized for endocrine disorders such as adrenal cortex insufficiency, autoimmune disorders like rheumatoid arthritis and systemic lupus erythematosus, severe dermatitis, allergic diseases such as bronchial asthma, serious ophthalmic inflammation, gastrointestinal diseases such as ulcerative colitis, and acute exacerbation of multiple sclerosis. It is also used for palliative treatment of malignant tumors such as leukemia and lymphoma and helps alleviate edema caused by nephrotic syndrome. This medication is effective in alleviating symptoms of various diseases and inhibiting disease progression, thus improving the overall health status of patients.
The initial dose may vary from 4mg to 48mg of methylprednisolone per day, depending on the symptoms of the specific disease. In general, low doses are sufficient for less severe conditions, but an initially high dose may be required in certain patients. The initial dose should be maintained or adjusted until a satisfactory response is obtained. If a satisfactory clinical result is not achieved after a reasonable period, discontinue treatment and switch to another appropriate therapy. The dose should be determined individually according to the various diseases being treated and the patient's response. After an adequate response is achieved, an appropriate maintenance dose should be determined by reducing the initial dose at suitable intervals until the minimum dose that maintains an adequate clinical response is reached. Continuous monitoring of the dose is required. Dose adjustment may be necessary in situations such as improvement or worsening of the disease, individual patient response to the medicine, and the effects on the patient of exposure to stressful circumstances that are not directly related to the symptoms of the disease being treated. In the latter case, it may be necessary to increase the dose for a period sufficient to achieve satisfactory control of the patient's condition. After long-term treatment, gradual dose reduction is recommended when discontinuing the medicine. For treatment of acute exacerbations of multiple sclerosis, administration of prednisolone 200mg daily for 1 week, followed by prednisolone 80mg every other day for 1 month, is effective (4mg of methylprednisolone is equivalent to 5mg of prednisolone). Alternate-day therapy means administering twice the daily dose on one morning during each 2-day period.
This medication should not be administered to certain patients. For example, patients who have allergies or hypersensitivity to this drug or its ingredients, and those with infections that are unresponsive to antibiotics or systemic fungal infections should not use this medication as it may suppress immune function and worsen infections. Additionally, it should not be given to patients with herpes simplex, herpes zoster, chickenpox, or those undergoing live vaccine administration, and it is also unsuitable for patients with lactose intolerance or specific genetic metabolic disorders. Caution is also necessary for patients who should be administered this drug carefully. Patients with glaucoma should be careful as their condition may worsen due to increased intraocular pressure, and patients with tuberculosis or keratitis may experience worsening symptoms due to immunosuppressive effects. Furthermore, patients with various conditions such as electrolyte imbalances, hypertension, thrombosis, a history of acute myocardial infarction, peptic ulcers, psychiatric disorders, and severe osteoporosis should be particularly cautious when using this medication, as symptoms may worsen in those with infections or diabetes. While taking this medication, various side effects such as edema, elevated blood pressure, muscle weakness, osteoporosis, gastrointestinal disturbances, skin changes, neurological symptoms, endocrine abnormalities, and increased ocular pressure may occur, so careful observation of symptoms is important. Long-term use may result in adrenal insufficiency, and abrupt discontinuation may lead to severe withdrawal symptoms, so the dosage should be gradually reduced. Additionally, there is an increased risk of infection, so special care should be taken to avoid exposure to infections like chickenpox or measles. This medication may interact with various other drugs, so dosage adjustments or caution may be necessary depending on the medications taken together. Attention should especially be paid when used in combination with heart rhythm-affecting drugs, antihypertensives, anticoagulants, diuretics, etc. Women who are pregnant or may become pregnant should consult with their doctor as this drug may pose risks to the fetus, and it is advised to avoid use during breastfeeding. Pediatric patients may have side effects such as growth retardation, so it should be administered cautiously at the minimum effective dose, and elderly patients are more susceptible to side effects such as infections, diabetes, and osteoporosis, thus requiring careful monitoring. Finally, it is important to discuss with healthcare professionals as the effectiveness of certain tests or vaccinations, such as vaccinations or skin tests, may be reduced while taking this medication.
