This medication is used to improve atherosclerotic symptoms in adult patients with ischemic stroke, myocardial infarction, or peripheral arterial disease. It also helps reduce complications due to cardiovascular issues in patients with acute coronary syndrome who have unstable angina or non-Q wave myocardial infarction and are undergoing or are scheduled to undergo medical therapy, percutaneous coronary intervention (PCI), or coronary artery bypass grafting (CABG). Additionally, it is used to reduce the risk of stroke and thromboembolism in patients with atrial fibrillation who have one or more vascular risk factors and where the use of vitamin K antagonists is not appropriate and the bleeding risk is low. In summary, this medication is used to enhance cardiovascular health by improving atherosclerosis and thrombus formation related to major vascular diseases.

How to take

Adult dose

For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, take 75 mg of clopidogrel orally once daily. For patients with acute coronary syndrome, start with a loading dose of 300 mg on the day of initiation and then maintain at 75 mg orally once daily. In this case, combine with aspirin at a dose of 75 - 325 mg once daily. For patients with atrial fibrillation, take 75 mg once daily.

Precautions

This medication helps blood flow by inhibiting the aggregation of platelets. It must be converted to its active form in the liver to be effective. Some people may genetically have a reduced function of the enzyme that activates this drug. In such cases, the efficacy of the drug may be diminished, leading to more frequent cardiovascular issues such as heart attack or stroke, and alternative treatment options should be considered for these patients. Additionally, medications that activate specific enzymes taken with this drug can increase the concentration of the active ingredient, raising the bleeding risk; therefore, concomitant use should be avoided. This drug should not be used if you are allergic to it, currently have a bleeding condition, have severe liver dysfunction, or are breastfeeding. Patients with a high risk of bleeding, such as those with a history of bleeding or abnormalities in blood coagulation, should use this medication with caution, and it should be discontinued 5-7 days prior to surgery and resumed after bleeding has stopped. Care should be taken when using this drug with other bleeding-risk medications, especially when combined with aspirin, as it may increase the risk of bleeding, and you should follow your doctor's instructions. If symptoms of bleeding (e.g., bruising, nosebleeds, bloody stools, etc.) occur while taking this medication, inform your healthcare provider immediately. The risk to the fetus during pregnancy has not been confirmed, but the risk of bleeding during delivery may be increased, so caution is advised. If you are breastfeeding, discuss with your doctor whether to discontinue breastfeeding due to the potential transfer of the medication into breast milk. Safety in children has not been established, so it's best not to use this medication in pediatric patients. Elderly patients or those with impaired kidney function may have a higher risk of bleeding, so careful monitoring and dosage adjustment is recommended. In case of overdose, bleeding time may prolong and bleeding may occur, and as there is no specific antidote, platelet transfusions may be necessary in case of bleeding. This medication should be kept out of reach of children, and transferring it to another container may alter its quality.
