Treatment of essential hypertension
Essential hypertension is treated to control blood pressure to a normal range, reducing the risk of cardiovascular diseases and maintaining the health of the heart and blood vessels. This medication helps lower blood pressure to prevent complications arising from hypertension and to improve the overall quality of life for patients. Therefore, it is used for essential hypertension.
For adults, the recommended initial dose is 10 mg once daily, regardless of meals. If blood pressure is not adequately controlled at this dose, it may be increased to 20 mg once daily, with a maximum of 40 mg per day if needed. Blood pressure-lowering effects will appear within 2 weeks after starting treatment, reaching maximal effect in about 8 weeks. If blood pressure is not controlled on monotherapy, combination therapy with a thiazide diuretic may be considered.
Safety and efficacy in pediatric patients have not been established.
This medication should not be used in women during the second and third trimesters of pregnancy, as it can cause serious harm or death to the fetus and newborn; therefore, treatment should be discontinued immediately upon confirmation of pregnancy. Do not use this medication in patients allergic to its ingredients, pregnant or nursing women, patients undergoing kidney dialysis, patients with severe liver dysfunction, patients with biliary obstruction, and patients with lactose intolerance. In patients with decreased blood volume or low salt levels, hypotension may occur, so it is important to check their condition carefully before starting treatment and take appropriate measures if hypotension occurs. For patients with impaired kidney function or kidney transplant recipients, there may be changes in kidney function when taking this medication, so regular blood testing to monitor kidney function and electrolyte levels is required. Due to the risk of hyperkalemia, patients with heart failure or kidney disorders should carefully monitor their potassium levels. Patients with aortic stenosis, mitral valve stenosis, or obstructive cardiomyopathy may experience worsening symptoms due to the blood pressure-lowering effects of this medication, so caution is necessary. Dizziness can occur when taking this medication, so avoid engaging in hazardous activities such as working from heights or driving. This medication may cause various side effects, including dizziness, headache, abdominal pain, diarrhea, muscle pain, rash, etc., but most are mild and temporary. If chronic diarrhea or weight loss occurs, consider the possibility of chronic malabsorption related to this medication and review the need to discontinue use. Caution is needed when taking with other antihypertensive medications, potassium-sparing diuretics, and potassium supplementation, as blood potassium levels may increase. When used with lithium, lithium levels can increase, leading to toxicity, so it is advisable not to use them together. Caution is also required when using non-steroidal anti-inflammatory drugs, as it may worsen kidney function. Taking this medication during pregnancy may result in severe abnormalities in the fetus; therefore, if pregnancy is confirmed, discontinue use immediately. It is recommended that nursing mothers either discontinue breastfeeding or stop this medication as it may be passed into breast milk. Safety and efficacy in pediatric patients have not been established, so it is advisable not to use this medication in children. For the elderly, the body concentration of this medication may increase; therefore, careful observation and dosage adjustment may be necessary. In cases of overdose, hypotension and palpitations may occur, and appropriate treatment should be administered based on the symptoms.
