Adult dosage
For Torsades de Pointes treatment, magnesium 8 mmol (1.6 g of magnesium chloride) is administered slowly as an IV bolus. Subsequently, magnesium 0.012 to 0.08 mmol (2.5 to 16.5 mg of magnesium chloride) is continuously infused. For acute hypokalemia with hypomagnesemia, magnesium 24 to 32 mmol (5 to 6.5 g of magnesium chloride) is administered daily along with potassium supplementation.
Children's dosage
In children, administer magnesium 0.1 to 0.3 mmol (20 to 60 mg of magnesium chloride) per kg over 24 hours.

This medication is used for the treatment of a cardiac arrhythmia known as Torsades de Pointes and helps improve acute hypokalemia associated with hypomagnesemia. Additionally, it supplements magnesium to maintain body balance during electrolyte imbalances. It is also used for magnesium supplementation in patients requiring parenteral nutrition and is effective in preventing and treating eclampsia (pregnancy-induced hypertension) during pregnancy. Due to these efficacies, it is widely used in managing heart health and pregnancy-related complications.

Dosage and Administration

Adult dosage

For Torsades de Pointes treatment, magnesium 8 mmol (1.6 g of magnesium chloride) is administered slowly as an IV bolus. Subsequently, magnesium 0.012 to 0.08 mmol (2.5 to 16.5 mg of magnesium chloride) is continuously infused. For acute hypokalemia with hypomagnesemia, magnesium 24 to 32 mmol (5 to 6.5 g of magnesium chloride) is administered daily along with potassium supplementation.

Children's dosage

In children, administer magnesium 0.1 to 0.3 mmol (20 to 60 mg of magnesium chloride) per kg over 24 hours.

Precautions

Caution is required when opening the ampule as glass fragments might enter. Special care is required, especially when used by children or elderly patients. When administering this injection, it should be done slowly, and care should be taken to avoid rapid infusion of magnesium. During injection, blood pressure, urine output, respiratory condition, and reflexes of the patient should be continuously monitored. Regular checks of magnesium levels in the blood should be performed, and treatment should be discontinued once the levels return to normal. This medication should not be used in conjunction with drugs containing calcium as they may interfere with each other's effects. Patients with poor kidney function, severe hypotension, heart failure, respiratory distress, coma, hypermagnesemia, or muscle weakness should not use this medication. Children and patients with heart diseases should take special precautions when using this medication. Pain or warmth may occur at the injection site, and rapid infusion in patients with severely impaired kidney function may result in dangerous hypermagnesemia. Caution is needed as use with certain medications may lead to increased drug concentration or heightened side effects. It should be used in pregnant women only if essential, as overdose may affect neonatal respiration. This medication can be passed through breast milk, hence, breastfeeding should be halted during treatment. In cases of overdose, symptoms such as decreased knee reflex, drowsiness, difficulty speaking, and respiratory distress may appear, with severe cases potentially leading to respiratory arrest or cardiac arrest. In cases of overdose, treatment may necessitate hydration, diuretics, artificial respiration, calcium injections, and in severe scenarios, blood dialysis. This medication is administered intravenously only for certain arrhythmias, and it should be slowly infused while the patient is lying down in a professional setting. It should be diluted with a glucose solution or normal saline, and should not be mixed with alcohol or alkaline solutions or calcium salts as precipitate may occur.

Side effects and when to seek care immediately

Serious side effects

  • Respiratory distress, respiratory arrest and cardiac arrest
  • Hypermagnesemia due to severe renal failure
  • Severe hypotension
  • Initial symptoms of hypermagnesemia may include cold sweat and pallor
  • Severe edema and blood pressure rise
  • Serious allergic reactions and cardiovascular abnormalities (including chest tightness)

Mild side effects

  • Pain, warmth, and vasodilation at the injection site
  • Decreased knee reflex, drowsiness, speech difficulties, muscle paralysis
  • Blood pressure fluctuations, changes in diuresis, etc.
  • Enhanced drug action due to increased plasma magnesium levels

Not everyone experiences these, and they can vary from person to person.

Storage

This medication should be stored in a sealed container to protect it from air and moisture, preferably at room temperature between 15 and 25 degrees Celsius. Storing it this way will help maintain the drug's effectiveness and prevent degradation.
