This medication is used to improve atherosclerosis-related symptoms in adult patients with ischemic stroke, myocardial infarction, or peripheral artery disease. It also helps alleviate cardiovascular abnormalities that may occur after drug treatment or coronary interventions, such as in patients with acute coronary syndrome, specifically unstable angina or non-Q wave myocardial infarction. Lastly, it plays a role in reducing the risk of stroke and thromboembolism in patients with atrial fibrillation who cannot take vitamin K antagonists. This medication is safe for patients with vascular risk factors and a low risk of bleeding, improving overall cardiovascular health.

How to take

Adult dose

1. For patients with ischemic stroke, myocardial infarction, or peripheral artery disease, take 75 mg of clopidogrel orally once a day. 2. For patients with acute coronary syndrome, initiate treatment with a loading dose of 300 mg once daily on the first day, followed by a maintenance dose of 75 mg once daily. Aspirin 75-325 mg should also be taken once daily alongside this medication. 3. For patients with atrial fibrillation,

Precautions

This medication is used to inhibit the aggregation of platelets in the blood to prevent the formation of clots, thereby preventing cardiovascular diseases such as myocardial infarction and stroke. This drug is converted into its active form by the enzyme CYP2C19 in the liver, and if the function of this enzyme is genetically diminished, the efficacy may decrease, necessitating a reevaluation of the treatment regimen. Additionally, co-administration with drugs that activate the CYP2C19 enzyme may increase the concentration of the active ingredient of this drug, raising the risk of bleeding, so combinations should be avoided. Patients with allergies to this drug or its components, those with bleeding disorders, severe liver damage, or nursing mothers should not take this medication. Patients with a high risk of bleeding, liver disease, kidney impairment, acute stroke, high blood pressure, the elderly, underweight patients, and those with hypersensitivity to thienopyridines should be treated cautiously. Especially if bleeding symptoms are suspected, appropriate testing including blood tests should be performed immediately, and in the case of scheduled surgeries, this medication should be discontinued 5-7 days prior and restarted only after hemostasis is achieved. This medication can cause side effects such as bleeding, thrombocytopenia, gastrointestinal symptoms (abdominal pain, dyspepsia, diarrhea), skin rash, headache, dizziness, abdominal pain, nausea, hypercholesterolemia, arthralgia, low back pain, purpura, epistaxis, depression, upper respiratory infections, rhinitis, bronchitis, cough, rash, pruritus, urinary tract infections, chest pain, weight loss, weight gain, facial flushing, fatigue, rigidity, facial swelling, fainting, constipation, vomiting, ulcer, appetite loss. Rarely, serious hematological adverse reactions due to thrombocytopenia or acquired hemophilia may occur, so if any abnormal symptoms arise, medical attention should be sought immediately. While taking this medication, the risk of bleeding may increase, and caution is required when used in conjunction with other drugs that affect blood coagulation (such as aspirin, warfarin, NSAIDs, SSRIs, etc.). During pregnancy, the effects on the fetus are unclear, so it should only be used when severe cardiovascular conditions require treatment, and it is advisable to discontinue before labor to reduce the risk of bleeding. In breastfeeding cases, because this medication may be secreted in breast milk, a choice should be made to either stop breastfeeding or discontinue the medication. Safety and effectiveness in pediatric patients have not been established, and elderly patients may face an increased risk of bleeding due to decreased kidney and liver function, so they should be treated cautiously. In cases of overdose, bleeding time may be prolonged, and since there is no specific antidote, treatments such as platelet transfusion may be necessary if bleeding is severe. This medication should be kept out of reach of children, and transferring it to other containers should be avoided.
