This medication is used for regional and infiltration anesthesia in dentistry. Regional anesthesia blocks the main branches of the nerves to provide anesthesia over a wide area, and infiltration anesthesia is performed by injecting directly into the tissue to anesthetize a local area. This effectively reduces pain during dental treatments and helps the patient receive treatment comfortably.

How it is given

Adult administration

For regional or infiltration anesthesia, use lidocaine hydrochloride as 6–40mg (0.3–2mL of this drug). For oral and maxillofacial surgery, use lidocaine hydrochloride as 60–100mg (3–5mL of this drug).

Precautions

This drug contains a component called sulfites, which can cause severe allergic reactions in sensitive individuals. Especially, life-threatening reactions such as anaphylaxis or asthma attacks may occur, so caution is required. This drug should not be used in certain patients. For example, it should be avoided in patients with intoxicated bleeding or in shock, patients with inflammation at the injection site, sepsis patients, and patients allergic to this drug or similar local anesthetics. Moreover, caution should be exercised when administering it to patients with hypertension, arteriosclerosis, heart failure, hyperthyroidism, diabetes, vascular spasms, or those taking tricyclic antidepressants or MAO inhibitors. This drug should also be used carefully in patients with central nervous system disorders, pregnant women, elderly patients, and patients with spinal deformities. Patients with liver or kidney issues or severe myasthenia gravis or acute porphyria also require careful management. During administration, abnormal reactions such as symptoms of shock, hypotension, respiratory depression, tremors, seizures, drowsiness, anxiety, dizziness, and nausea should be closely monitored. If these symptoms appear, administration must be stopped immediately, and appropriate treatment should be administered. Cardiovascular symptoms such as bradycardia or hypotension and cardiac arrest may occur, so caution is necessary. Additionally, hypersensitivity reactions such as skin rashes or edema have also been reported. When using this drug, it is important to always be prepared for emergencies and to have first aid readily available. Administer the smallest possible dose slowly and ensure that the injection site does not enter the blood vessel. When used in combination with other drugs, interactions may occur, so it is essential to consult medical professionals, especially when taking antiarrhythmics, CYP450 3A4 inhibitors, or blood pressure enhancers. Women who are pregnant or breastfeeding should carefully consider the risks and benefits of administering this drug, as it may pass into breast milk and affect the infant. Elderly patients may be more sensitive to this drug, so dose adjustment and condition monitoring are needed. In case of overdose, seizures and respiratory distress can occur, requiring immediate airway management, oxygen support, and appropriate medication. In severe cases, cardiopulmonary resuscitation may be necessary. Lastly, this drug is not intended for ophthalmic use, and sufficient research on its carcinogenicity or reproductive toxicity has not been conducted, so caution is advised.
