Improvement of atherosclerotic symptoms; prevention of thromboembolism associated with atrial fibrillation
This medicine helps alleviate symptoms in adult patients with atherosclerotic diseases such as ischemic stroke, myocardial infarction, and peripheral artery disease. It is used to improve related symptoms due to cardiovascular complications in patients with acute coronary syndrome, especially those treated with drug therapy, coronary intervention, or coronary artery bypass surgery for unstable angina or non-ST elevation myocardial infarction. Additionally, in patients with atrial fibrillation with vascular risk factors who find it difficult to use vitamin K antagonists and have a low risk of bleeding, this drug can reduce the risks of stroke and thromboembolism. In summary, this drug is effective in improving symptoms of atherosclerotic diseases and reducing the risk of cardiovascular complications and thrombo-related issues, as well as preventing stroke.
○ Adults 1. In patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, administer clopidogrel 75 mg orally once daily. 2. In patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), begin with a loading dose of 300 mg of this medicine once on the day treatment is initiated, followed by a maintenance dose of 75 mg orally once daily. In this case, aspirin 75 ~ 325 mg should be administered concomitantly with this medicine once daily. 3. In patients with atrial fibrillation, administer 75 mg of this medicine orally once daily. In this case, aspirin 75 ~ 100 mg should be administered concomitantly with this medicine once daily. ○ Patients with renal impairment: Treatment experience in patients with renal impairment is limited. ○ Patients with hepatic impairment: Treatment experience in patients with moderate hepatic disease and a bleeding diathesis is limited. This medicine may be administered with or without food.
This drug is intended to prevent the aggregation of platelets in the blood, thereby preventing the formation of thrombus. Individuals with genetically reduced function of a specific enzyme (CYP2C19) may experience decreased effectiveness of this drug, and in such cases, alternative treatment options should be discussed with a physician. Taking this drug alongside medications that activate the CYP2C19 enzyme may increase the risk of bleeding, so it is advisable not to combine them. This drug should not be used by patients allergic to this medication or its components, patients with bleeding, patients with severely impaired liver function, nursing women, or those with specific genetic diseases. Patients at high risk of bleeding or those scheduled for surgery should use this drug cautiously and may need to discontinue use 5 to 7 days before surgery, if necessary. Since this drug can cause bleeding, any symptoms of bleeding should be reported to a doctor immediately. There may be an increased risk of bleeding when taken with other bleeding risk medications (e.g., warfarin, aspirin, non-steroidal anti-inflammatory drugs, selective serotonin reuptake inhibitors, etc.). It is recommended that pregnant women discontinue use before delivery due to the risk of bleeding, and if nursing, they should consult a doctor about whether to stop breastfeeding due to possible secretion of the drug in breast milk. Safety in children has not been established, so it is advisable not to use it for them. Elderly patients should take this medication cautiously under a doctor's guidance due to decreased bodily function and a higher risk of bleeding and other side effects. Overdose can increase the risk of bleeding, and there is no specific antidote, so platelet transfusions may be necessary to stop the bleeding. This drug should be kept out of the reach of children and should not be transferred to other containers.
