This medication helps improve atherosclerotic symptoms in patients with stroke, myocardial infarction, or peripheral artery disease. It also improves atherosclerotic symptoms caused by cardiovascular abnormalities in patients with acute coronary syndrome, especially those with unstable angina or non-ST elevation myocardial infarction, reducing the risk of death, myocardial infarction, stroke, or refractory ischemia. Additionally, it is used to reduce the risk of stroke and thromboembolism in patients with atrial fibrillation who have vascular risk factors and are at low risk of bleeding while unable to take vitamin K antagonists. This effectively improves the risks associated with various cardiovascular diseases related to atherosclerosis and thrombosis.

How to take

Adult dose

For patients with ischemic stroke, myocardial infarction, or peripheral artery disease, administer clopidogrel as 75 mg orally once daily. For patients with acute coronary syndrome (unstable angina or non-ST elevation myocardial infarction), start with a loading dose of 300 mg orally once daily on the first day of treatment and then maintain with 75 mg orally once daily thereafter. At this time, aspirin 75 to 325 mg should be administered daily.

Precautions

This medication prevents the aggregation of platelets, preventing blood from clotting. Patients with genetically reduced function of a specific enzyme (CYP2C19) may experience reduced effectiveness of this medication, and in such cases, alternative treatment methods should be considered. Furthermore, using medications that activate this enzyme together with this medication may increase the concentration of the drug and raise the risk of bleeding; therefore, it is advisable not to take them together. This medication should not be used in individuals with hypersensitivity, those with bleeding, severe liver damage, nursing women, or individuals with lactose intolerance. Patients at high risk of bleeding or those scheduled for surgery should use this medication cautiously and may need to discontinue it 5 to 7 days before surgery. There is an increased risk of bleeding when this medication is taken with other bleeding-risk medications, so caution is advised. If any bleeding symptoms occur during treatment, it is essential to inform a doctor immediately and to notify medical professionals of the use of this medication before surgery or starting new medications. It is important to regularly check for bleeding or blood abnormalities through blood tests while taking this medication. Additionally, thrombocytopenia or severe blood disorders may occur rarely, so if symptoms of infection or unusual symptoms arise, consult healthcare professionals immediately. This medication may cause gastrointestinal disturbances, skin rashes, headaches, and dizziness, and rarely serious bleeding or allergic reactions may occur. While the risk to the fetus during pregnancy is low, it is recommended to discontinue the medication before delivery due to the risk of bleeding at childbirth. Nursing women should consider stopping breastfeeding or discontinuing the medication as it is uncertain whether this medication is secreted in breast milk. The safety and efficacy in children have not been established, and elderly patients should be observed carefully due to a higher risk of bleeding during use. In case of overdose, bleeding time may be prolonged, and the risk of bleeding may be increased, necessitating measures such as platelet transfusion.
