Improvement of essential hypertension
Essential hypertension is an important condition for maintaining blood pressure within the normal range to reduce the risk of cardiovascular diseases. This medication helps lower blood pressure and relieve the burden on the heart and blood vessels, improving health. By maintaining consistent usage, it can also contribute to the prevention of complications. Thus, it is used for the treatment of essential hypertension.
The recommended initial dose is 10 mg once daily, administered regardless of meals. If blood pressure is not adequately controlled at this dosage, it can be increased to 20 mg once daily. If further reduction in blood pressure is required, the maximum dosage can be increased to 40 mg once daily. The antihypertensive effect appears within two weeks after treatment initiation, with maximum effect occurring after approximately eight weeks. If blood pressure is not controlled with monotherapy, combination therapy with thiazide diuretics such as Hydrochlorothiazide may be considered. For better compliance, it is recommended to take it at the same time each day (e.g., in the morning). In patients at risk for intravascular volume depletion (e.g., in patients taking diuretics, especially those with renal impairment), it is recommended to start treatment at a low dose while adequately monitoring their status. <For Elderly> No dose adjustment is necessary when administering this medication to elderly patients aged 65 and older. There were no overall differences observed in efficacy and safety between elderly and younger patients; however, it cannot be excluded that some elderly patients may demonstrate more sensitive reactions. <For Patients with Renal Impairment> The maximum dosage for patients with mild to moderate renal impairment (creatinine clearance 20-60 mL/min) is 20 mg once daily. <For Patients with Liver Impairment> Patients with mild liver impairment do not require dose adjustment. For patients with moderate liver impairment, the recommended initial dose is 10 mg once daily, which can be increased up to a maximum of 20 mg once daily. There is no experience with the use in patients with severe liver impairment; hence, the use of this drug is not recommended. <For Pediatric Patients> Safety and efficacy in pediatric patients have not been established.

This drug may cause serious harm or death to the fetus and newborn if taken during the second and third trimesters of pregnancy, so it must be immediately discontinued upon confirmation of pregnancy. It should not be used by patients who are allergic to any of its components, are pregnant or breastfeeding, are undergoing renal dialysis, have severe liver disease, have bile duct obstruction, or have certain genetic lactose intolerance. Patients with low blood volume or low salt, those with reduced kidney function, kidney transplant recipients, patients with renovascular hypertension, those at risk for hyperkalemia, patients with aortic or mitral stenosis, and those with primary aldosteronism should use this drug with special caution. This medication may cause dizziness; hence, it is advisable to avoid hazardous activities such as driving or operating machinery. Patients with hypertension may commonly experience mild side effects like dizziness, and there have been rare reports of chest pain, peripheral edema, abdominal pain, headache, rash, etc. If symptoms of gastrointestinal issues, such as chronic diarrhea, appear, the medication should be discontinued immediately, and a specialist should be consulted. Caution is needed if used in combination with potassium-sparing diuretics or potassium supplements as it can increase blood potassium levels, and it can enhance the blood pressure-lowering effect when combined with other antihypertensives. Lithium preparations should not be used concurrently as they may increase lithium levels and cause toxicity. Kidney function should be periodically monitored when used in conjunction with non-steroidal anti-inflammatory drugs as it may worsen renal function. Use during pregnancy poses significant risks to fetal health, and it is also recommended to stop breastfeeding or discontinue use during lactation, as this drug may be transmitted through breast milk. Safety in children has not been established, so it is not advisable to use in pediatric patients. Elderly patients may have increased drug concentrations in the body, so caution is necessary when administering. Overdose may result in hypotension, palpitations, bradycardia, etc.; therefore, if any symptoms occur, it is essential to inform healthcare professionals immediately for appropriate treatment.
