This medication is used to control blood pressure in patients with essential hypertension. It also helps reduce the risk of myocardial infarction, stroke, and death caused by cardiovascular diseases in patients aged 55 and older who have a history of coronary artery disease, peripheral artery disease, stroke, transient ischemic attacks, or high-risk diabetes. Therefore, this drug is effective not only for hypertension management but also for reducing the risk of major cardiovascular diseases. This treatment improves cardiovascular health and contributes to the patient maintaining a healthier lifestyle.

How to take

Adult dose

As telmisartan, 40 mg should be taken orally once daily with water, regardless of meals. In some patients, 20 mg once daily may be effective, and if necessary, the dose may be increased to 80 mg once daily. For reducing the risk of cardiovascular disease, it is recommended to take 80 mg orally once daily with water. In patients with liver impairment, the dose should not exceed 40 mg once daily.

Precautions

This medication can cause serious harm, particularly to the fetus and newborn, during pregnancy; therefore, it should be discontinued immediately if pregnancy is confirmed. It should not be administered to individuals with allergies to this drug or its components, pregnant or breastfeeding women, individuals with severe liver dysfunction, patients with hereditary angioedema, and patients with certain kidney disorders. Care should be taken when prescribing this medication to the elderly, patients with hyperkalemia, liver disease, heart valve disorders, and ischemic heart disease, as there is the risk of excessively lowering blood pressure. During treatment, blood pressure, kidney function, and electrolyte levels should be regularly monitored to detect and address adverse reactions early. Although this drug is effective in lowering blood pressure, it may rarely cause serious side effects such as angioedema, worsening kidney function, liver dysfunction, hyperkalemia, hypoglycemia, interstitial pneumonia, and rhabdomyolysis; if these symptoms appear, the physician should be informed immediately. Diabetic patients should observe for symptoms of hypoglycemia closely, and if angioedema symptoms (swelling of the face, lips, pharynx, or tongue) occur, the medication should be stopped immediately, and treatment should be sought. There may be interactions with other antihypertensive medications; thus, it is essential to inform the doctor or pharmacist about any other medications being taken. Patients undergoing hemodialysis or those with low sodium levels or blood volume are at risk of hypotension, so their condition should be corrected before administration, and they should be carefully monitored afterward. Women who are pregnant or planning to become pregnant should avoid using this medication, and breastfeeding women should either avoid this medication or discontinue breastfeeding. Safety and efficacy have not been established in children under 18 years; therefore, it is not recommended for this age group. Elderly patients should be cautious as excessive blood pressure lowering may increase the risk of stroke and other complications. In case of overdose, symptoms such as hypotension, dizziness, and acute renal failure may occur; immediate medical attention should be sought for appropriate treatment. This medication should be stored out of the reach of children and kept sealed until used.
