Adult dose
1. For patients with ischemic stroke, myocardial infarction or peripheral arterial disease, clopidogrel should be administered orally at a dose of 75 mg once daily. 2. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), a loading dose of 300 mg should be administered orally once on the day of initiation, followed by a maintenance dose of 75 mg once daily. At this time, aspirin 75–325 mg should be co-administered once daily.

This medication is used to improve atherosclerotic symptoms in adults with ischemic stroke, myocardial infarction, or peripheral arterial disease. It also helps prevent cardiovascular complications and improve atherosclerotic symptoms in patients with acute coronary syndrome or those who have undergone coronary interventional procedures or coronary artery bypass surgery. Furthermore, it reduces the risk of stroke and thromboembolism in patients with atrial fibrillation who have vascular risk factors and for whom Vitamin K antagonists are not appropriate. Overall, it helps reduce the risk of cardiovascular disease and enhance health.

How to take

Adult dose

1. For patients with ischemic stroke, myocardial infarction or peripheral arterial disease, clopidogrel should be administered orally at a dose of 75 mg once daily. 2. For patients with acute coronary syndrome (unstable angina or non-Q-wave myocardial infarction), a loading dose of 300 mg should be administered orally once on the day of initiation, followed by a maintenance dose of 75 mg once daily. At this time, aspirin 75–325 mg should be co-administered once daily.

Precautions

This medication helps prevent platelet aggregation, allowing blood to flow more easily. People genetically deficient in the enzyme CYP2C19, which activates this medication, may experience reduced effectiveness, and in such cases, alternative treatments should be discussed with a physician. Additionally, concomitant use with drugs that activate the CYP2C19 enzyme may increase the active ingredient concentration of this medication, raising the risk of bleeding; thus, such combinations are not recommended. Patients with allergies to this medication or its components, those with severe bleeding, very poor liver function, nursing mothers, or individuals with specific genetic disorders such as lactose intolerance should not take this medication. Those with allergies to soybean oil, soy, or peanuts should use this medication with caution. Patients at high risk of bleeding should notify their doctor immediately if they experience any bleeding symptoms while on this medication, and if surgery is planned, it should be discontinued 5–7 days prior to the procedure. This medication may increase the risk of bleeding when taken with other antiplatelet agents, anticoagulants, non-steroidal anti-inflammatory drugs (NSAIDs), and selective serotonin reuptake inhibitors (SSRIs), so caution is advised. During treatment with this medication, gastrointestinal bleeding, intracranial bleeding, and various bleeding events may occur; hence, if any bleeding symptoms arise, immediate consultation with healthcare professionals is necessary. Rarely, side effects such as reduced blood cell counts may occur, so it is advisable to get tested if fever or signs of infection develop. While this medication does not have a significant impact on the fetus during pregnancy, there is a risk of bleeding during delivery; thus, it is recommended to discontinue use before delivery. If breastfeeding, consultation with a physician regarding whether to continue or discontinue breastfeeding is necessary due to the potential for this medication to be excreted in breast milk. The safety and efficacy of this medication in children have not yet been established, so it should be used cautiously in pediatric patients. Elderly patients may have decreased bodily functions, increasing the risk of side effects such as bleeding, so dose adjustments and monitoring of condition are necessary. In cases of overdose, bleeding may worsen, and as there is no specific antidote, immediate medical attention is required. This medication should be stored out of reach of children and not transferred to other containers.
