This medication is used to reduce blood pressure in patients with essential hypertension. Additionally, it helps reduce the risk of myocardial infarction, stroke, and death from cardiovascular disease in patients aged 55 and older with a history of coronary artery disease, peripheral artery disease, stroke, or transient ischemic attack, or with clinical evidence of end-organ damage from high-risk diabetes. It is particularly offered as an alternative for patients who cannot use ACE inhibitors and is effective in reducing the overall risk of cardiovascular disease. Therefore, this medication is used for the management of hypertension as well as maintaining cardiovascular health.

How to take

Adult dose

Essential hypertension: Telmisartan 40 mg is administered orally once daily with water, regardless of meals. In some patients, 20 mg once daily may be effective. If necessary, the dose can be increased up to 80 mg once daily. Dose increases show maximum antihypertensive effect 4 to 8 weeks after starting treatment. Reduction of cardiovascular disease risk: 80 mg once daily is recommended.

Precautions

This medication can cause serious harm to the fetus and newborn if taken during pregnancy, so it should be discontinued immediately upon confirmation of pregnancy. Patients with allergic reactions to this medication or a history of severe liver impairment, bile duct obstruction, or hereditary angioedema should not use it. Certain patient groups such as the elderly, patients with hyperkalemia, and those with mild to moderate liver impairment should use it with caution, especially if there are underlying heart or kidney diseases. Regular monitoring of blood pressure and kidney function is necessary as blood pressure may become too low or kidney function may decline during treatment with this medication. Care should be taken when consuming potassium-rich foods or potassium supplements as there is an increased risk of elevated potassium levels in the blood. This medication may interact when used with other antihypertensives or specific drugs, so all current medications should be disclosed to healthcare providers and regular monitoring may be needed. Caution is advised during driving or operating machinery as dizziness or fainting may occur. Use of this medication is not recommended for children, pregnant women, or breastfeeding women. In case of an overdose, hypotension, dizziness, or acute renal failure may occur, and immediate medical assistance should be sought. This medication should be stored out of reach of children, and it is advisable to open the sealed package just prior to use.

Urticaria, severe swelling (including angioedema and facial swelling), chest tightness, difficulty breathing, cold sweat, pallor, severe allergic reactions, severe rashes with high fever, blisters, skin peeling, pain in oral/ocular mucosa, severe skin reactions, shortness of breath or difficulty breathing, asthma attacks, respiratory abnormalities associated with cough and fever, jaundice, liver dysfunction, sudden and severe headache, lightning headache, seizures, altered consciousness.
