This medication is used to manage essential hypertension and also assists in the standard treatment of elderly patients (aged 70 and over) with stable chronic heart failure. It is used to stabilize blood pressure and improve heart function.

How to take

Adult dose

For essential hypertension, administer 5 mg orally once daily at the same time each day. The antihypertensive effect can generally be observed 1 to 2 weeks after starting treatment, and in some cases, effects may be seen after 4 weeks. It can be taken with food. For patients with renal impairment, the initial dose is 2.5 mg orally once daily, which can be increased to 5 mg as needed. For older adults (65 years and over), the initial dose is 2.5 mg orally once daily, which can be increased to 5 mg as required. For patients aged 70 and over with chronic heart failure, start at 1.25 mg once daily.

Precautions

This medication should not be used in certain patients. For example, it should not be administered to patients who are allergic to this medication, have bronchial asthma, metabolic acidosis, bradycardia, or severe heart failure or hypotension. Additionally, it is not suitable for patients with severe liver or kidney impairment, pregnant or breastfeeding women, and patients with specific genetic lactose intolerance. Caution is required when administering this medication to patients with chronic lung disease or heart failure, peripheral circulation disorders, thyroid dysfunction, and diabetes. Older adults should also be monitored carefully. During treatment, side effects such as skin rash, changes in vision, abnormal heart rhythms, headaches, dizziness, digestive issues, shortness of breath, and fatigue may occur, so any unusual symptoms should be reported to a healthcare professional immediately. It may be necessary to discontinue this medication before surgery, and abrupt discontinuation can worsen heart problems, so it must be reduced gradually according to a physician's instructions. This medication can interact with other drugs, so special caution is needed when taken in conjunction with calcium channel blockers, antiarrhythmics, central nervous system medications, and diabetes medications. If pregnant or breastfeeding, it should be avoided as it can cause side effects in the fetus or newborn. Safety and effectiveness have not been confirmed for children under 18 years, so it is not recommended. Elderly patients should start at a low dose and be administered cautiously; dosage should be gradually adjusted as necessary. In case of overdose, severe bradycardia, hypotension, and respiratory distress may occur, requiring immediate medical treatment.
