This medication is used to improve symptoms related to atherosclerosis, such as ischemic stroke, myocardial infarction, and peripheral arterial disease. It also reduces the risk of cardiovascular events such as death, myocardial infarction, and stroke in patients with acute coronary syndrome, especially those with unstable angina or non-Q wave myocardial infarction undergoing medical treatment, coronary intervention, or coronary artery bypass surgery. Furthermore, in adult patients with atrial fibrillation who have one or more vascular risk factors and in whom anticoagulation with vitamin K antagonists is challenging and bleeding risk is low, it helps decrease the risk of stroke and thromboembolism. This medication improves the prevention and treatment of such cardiovascular diseases.

How to take

Adult dose

For patients with ischemic stroke, myocardial infarction, or peripheral arterial disease, clopidogrel is administered orally once daily at a dose of 75 mg. For patients with acute coronary syndrome (unstable angina or non-Q wave myocardial infarction), the initial dosage is 300 mg once as a loading dose on the first day, followed by a maintenance dose of 75 mg daily. At this time, aspirin 75 ~ 325 mg should be administered daily concurrently with this medication.

Precautions

This medication prevents platelets from aggregating, reducing blood clotting. Its efficacy may be diminished in individuals genetically impaired in the function of a specific enzyme (CYP2C19); in such cases, adjusting the therapeutic approach or switching to another medication may be necessary. Furthermore, co-administration with drugs that activate this enzyme may increase drug concentration, raising the risk of bleeding, so this combination is not recommended. Patients with allergies to this medication, severe bleeding, severely impaired liver function, breastfeeding women, and lactose intolerant patients should not take this medication. Particularly, those with allergies to soybean oil, soy, and peanuts should exercise caution when taking this medication. Patients at high risk for bleeding or those scheduled for surgery should consult a physician before taking this medication, and should seek immediate medical attention if bleeding symptoms occur. The risk of bleeding may increase when this medication is taken concurrently with other antiplatelet drugs. During treatment, bleeding may take longer to stop; if a bleeding area is present or bleeding continues, promptly inform healthcare professionals. Rarely, this medication may cause thrombocytopenic purpura (TTP), a serious blood disorder, so immediate treatment is warranted if symptoms such as thrombocytopenia, anemia, neurological symptoms, or fever occur. While taking this medication, side effects such as headache, dizziness, abdominal pain, diarrhea, and skin rashes may occur; if symptoms are severe, consult a physician. Elderly patients, as well as those with impaired renal or liver function, are at higher risk for bleeding and other side effects, necessitating additional caution during treatment. If pregnant or breastfeeding, consult a physician before taking this medication, as it may affect the fetus or infant. In cases of overdose, the risk of bleeding may significantly increase, and treatments such as platelet transfusions may be required. This medication has not been adequately studied for safety and efficacy in children and should be avoided in this population. During treatment, inform a physician or pharmacist before starting any new medications, and report the use of this medication prior to any surgical or dental procedures.
