This medication is used in patients with essential hypertension and heart failure. It is particularly effective in heart failure patients who are intolerant to ACE inhibitors. Additionally, it helps reduce the risk of mortality in clinically stable patients with left ventricular failure or dysfunction following myocardial infarction. Therefore, this medication is usefully applied in the treatment of conditions related to hypertension and decreased cardiac function.

How to take

Adult dose

For essential hypertension, it is recommended to take 80 mg orally once daily with water during meals or in between meals, preferably at the same time each day. In cases where blood pressure control is difficult, the dose may be increased to 160 mg per day and may go up to a maximum of 320 mg. Elderly patients (75 years and older) should start at 40 mg once daily. Adjustments should be made for patients with renal impairment, fluid depletion, or hepatic impairment.

Precautions

This medication may cause serious harm to the fetus if taken during pregnancy, especially in the second and third trimesters. Therefore, it should be discontinued immediately upon confirmation of pregnancy. Women who are pregnant or may become pregnant, nursing mothers, patients with severe liver impairment, patients with severe kidney impairment, and patients with certain hereditary angioedema should not take this medication. Elderly patients, patients with hyperkalemia, patients with liver impairment, patients with heart valve diseases, and patients at risk of a sudden drop in blood pressure should use this medication cautiously. Side effects such as dizziness, hypotension, fatigue, headache, cough, etc., may occur during administration; in severe cases, kidney damage or hyperkalemia may occur, so symptoms should be closely monitored. A sudden drop in blood pressure may occur, so it is recommended to start with a low dose at the beginning and gradually increase while monitoring the patient's condition. This medication may interact with other blood pressure medications, diuretics, and potassium-sparing agents; therefore, it should be taken after consultation with a doctor or pharmacist, especially due to the risk of elevated potassium levels. It is advisable that pregnant or nursing mothers do not take this medication; if it must be taken, consulting a specialist and discontinuing nursing is recommended. The safety and efficacy of this medication have not been established in children and adolescents under 18 years of age, so usage should be avoided. In case of overdose, serious symptoms such as hypotension, altered consciousness, and shock may occur, so immediate medical attention is required.
