Treatment of erectile dysfunction
This medication is used for the treatment of erectile dysfunction. Erectile dysfunction is one of the male sexual dysfunctions, and this drug helps to improve blood flow for an erection. It is available in various formulations such as tablets, oral disintegrating films, chewable forms, and powder forms to enhance patient convenience. Regular use can be effective in improving sexual function and it can be used safely. Therefore, it is a very useful medication for treating erectile dysfunction.
Administer sildenafil orally at the recommended dose of 25 ∼ 50 mg once daily, approximately 1 hour before sexual activity. In some cases, it may be administered 4 hours to 30 minutes before sexual activity. The dose may be increased or decreased according to efficacy and tolerability. In the following patients, adjust the initial dose to 25 mg because sildenafil plasma concentrations may be increased: - Age 65 years or older (AUC increased by 40 %) - Hepatic impairment (cirrhosis: 80 %) - Severe renal impairment (creatinine clearance of 30 mL/min or less: 100 %) - Concomitant use with CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, erythromycin (182 %), saquinavir (210 %)); increased blood concentrations may increase efficacy and the incidence of adverse reactions. - In patients receiving concomitant ritonavir, do not exceed 25 mg as a maximum single dose in any 48-hour period.
This drug may cause a significant drop in blood pressure when taken with nitrate preparations or NO donors, so the patient should be checked by a doctor to ensure they are not taking these medications before use. Patients with cardiovascular diseases should thoroughly assess their condition before using this medication, and severe cardiovascular patients should avoid its use as it may put stress on the heart. Sexual activity itself may burden the heart, so patients with poor heart condition should avoid using this medication or engaging in sexual activity. This medication may temporarily lower blood pressure by dilating blood vessels; thus, patients with underlying cardiovascular diseases should exercise special caution. There is no established safety for patients with specific heart conditions, extremely low or high blood pressure, pigmentary retinopathy, and they should be cautious. If an erection lasts longer than 4 hours or there is pain, immediate medical attention is required as neglecting this may cause penile tissue damage and loss of erectile function. Patients with left ventricular outflow obstruction or severely impaired blood pressure control may be more sensitive to the vasodilatory effects of this medication. When used in conjunction with certain antiviral medications, the blood concentration of this medication can significantly increase, requiring dose adjustment and caution for patients taking these drugs. The artificial sweetener aspartame included in this drug should not be administered to patients with phenylketonuria. Patients with known allergies to this medication or its components, those taking nitrate preparations, severe cardiovascular disease, severe liver or kidney failure, children under the age of 18, and women should not use this medication. Patients with specific blood pressure anomalies or those who have recently experienced a stroke or myocardial infarction should also not be administered this drug. Cautious use is required for patients with anatomical penile deformities or bleeding disorders, elderly patients, those with severe kidney or liver failure, and patients on CYP3A4 inhibitors. If cardiovascular symptoms occur during sexual activity, immediately discontinue use and consult a physician; take caution as hypotension symptoms may develop due to vasodilatory effects. Visual disturbances or dizziness may occur when operating vehicles or machinery; therefore, caution is advised. Concurrent use with other erectile dysfunction treatments is not recommended as safety has not been confirmed. In case of overdose, headache, flushing, dizziness, dyspepsia, visual disturbances, etc. may worsen significantly, and prompt treatment is required as removal of the drug by kidney dialysis is difficult. This medication should be kept out of reach of children and it is advisable not to transfer it to another container. Expectant mothers or nursing women cannot take this medication, and its use is prohibited in women. Elderly patients may have increased blood concentration of this drug, so it is safe to start with a low dose initially. Possible adverse reactions that may occur while taking this medication include headache, flushing, dyspepsia, dizziness, and visual disturbances, and immediate consultation with a physician is necessary if serious symptoms develop.
