Precautions

This medication should not be used in certain patients. Those allergic to this drug or its components should not take it. Patients with gastrointestinal or bowel obstruction should avoid taking this medicine, as it may exacerbate symptoms by inhibiting bowel movement. Patients with weak gastric muscles or relaxed intestines may also experience worsened symptoms due to the action of this drug. Patients with urinary retention may have worsened symptoms due to the decreased contraction of bladder muscles caused by this medication. Patients with angle-closure glaucoma are at risk of worsening symptoms due to increased intraocular pressure. Those with severe myasthenia gravis may experience worsened symptoms due to the anticholinergic effects of this medication. Patients with severe heart disease should be cautious as side effects may include arrhythmias. Toxic megacolon or ulcerative colitis patients may see symptoms worsen with this medication. Patients with moderately impaired liver function or worse should also be cautious with this medication. Patients with a history of arrhythmias or those with arrhythmias may experience worsened symptoms due to this medication. Moreover, patients with dysuria should be aware that this medication may further worsen symptoms or increase post-void residual urine. Patients with weak liver or kidney function may have slow metabolism and excretion of this drug, increasing the risk of side effects. Patients with Parkinson’s disease or cerebrovascular disorders may experience worsened symptoms or changes in mental state. Those with hyperthyroidism may experience exacerbated symptoms such as increased heart rate due to this medication's effects. Elderly patients are at higher risk of liver and kidney dysfunction, so starting with a lower dose is safer. Patients with autonomic nervous system disorders, coronary artery disease, severe congestive heart failure, tachycardia, prostate enlargement, cerebral sclerosis, and gastroesophageal reflux should also use caution. Glaucoma patients should use this medication carefully as symptoms may worsen due to increased intraocular pressure. This medication affects the central nervous system, which can lead to hallucinations or delirium, so symptoms should be closely monitored. Rarely, gastrointestinal side effects such as bowel obstruction may occur, prompting attention to symptoms like constipation or abdominal distension. Cardiac-related side effects may include palpitations, irregular heartbeats, and bradycardia, necessitating careful observation. Hematological effects may involve decreased platelet count, renal impairment, and urinary retention symptoms. Cutaneous reactions may include rashes, itching, and in severe cases, serious skin reactions like Stevens-Johnson syndrome. Increases in intraocular pressure may lead to acute glaucoma attacks, necessitating attention to symptoms like eye pain or diminished vision. Liver function abnormalities and musculoskeletal issues like myalgia or rhabdomyolysis have also been reported. Accurate diagnosis is necessary when taking this medication, and care should be taken to avoid confusion with other diseases such as bladder cancer, prostate cancer, or urinary tract infections. Treatment of underlying conditions such as prostate enlargement must take precedence in patients with lower urinary obstruction. Eye control disorders, drowsiness, and dizziness may occur, so it is advisable to avoid driving or operating dangerous machinery. Considering the increased risk of side effects, caution is advised when taken in combination with anticholinergics, tricyclic antidepressants, phenothiazine medications, or MAO inhibitors. Pregnant women or women of childbearing age should avoid this medication due to the established safety profile being unclear; nursing mothers should also ideally stop breastfeeding while taking this medication. Safety has not been established for children, so its use in this population is not recommended. Overdose may result in hallucinations, seizures, paralytic ileus, urinary retention, cardiac abnormalities, etc., requiring immediate medical treatment.
