Treatment of acute renal failure and removal of water-soluble toxins (continuous renal replacement therapy).
This medication is used for patients who require continuous renal replacement therapy due to acute renal failure. Additionally, it can be utilized as a fluid substitute or dialysis solution during hemodialysis and blood filtration, aiding in the removal of water-soluble toxins. It is particularly suited for patients with normal blood potassium levels and effectively improves toxin elimination while supporting renal function during treatment. Therefore, it is very useful for the treatment of acute renal failure and its related symptoms.
In adults, it should be administered under the guidance of a qualified physician. 1) In the case of acute renal failure, a dosage of 2,000 mL/h is appropriate based on a patient weighing 70 kg, depending on the patient's metabolic level to facilitate waste removal. This drug should not be used in certain patients. It should not be administered to patients allergic to any component of this medication or those with elevated blood potassium levels, as well as patients with metabolic alkalosis. Additionally, patients in a state of excessive metabolism or who do not have sufficient blood flow, as well as those at high risk of bleeding, should not use this drug. When using this drug, various side effects may occur. For example, gastrointestinal issues such as nausea or vomiting, fluctuations in blood pressure, muscle cramps, etc., may arise. Additionally, symptoms such as shortness of breath or leg swelling may manifest due to excess or deficiency of bodily fluids, so caution is necessary. During treatment, it is important to periodically check potassium levels in the blood, and appropriate measures should be taken if the potassium level is too low or too high. Sodium levels should also be regularly tested to prevent side effects due to rapid changes. Calcium, magnesium, phosphate, glucose, acid-base balance, as well as body weight and fluid status should be continuously monitored to identify any abnormal signs early. This drug may not adequately replenish important nutrients during hemodialysis or blood filtration, so appropriate nutritional supplementation may be necessary if needed. When used with other medications, interactions may occur; thus, it is essential to adhere to dosage guidelines and closely monitor the patient's condition. In particular, caution should be exercised for signs of toxicity when used with medications such as digitalis. Pregnant or breastfeeding women should avoid this drug unless specifically required, as its safety has not been thoroughly established. There is insufficient data on the safety and efficacy of this drug in children, so its use is not recommended. In the case of overdose, imbalances in bodily fluids and electrolytes may occur, so prompt adjustment or discontinuation of treatment should be implemented upon symptom onset. Before use, be sure to mix the two solutions and warm them to a temperature similar to body temperature before infusion, ensuring the solution is clear and free of foreign substances. During the use of this drug, white precipitate may form in the infusion tube, so it should be checked regularly, and if any precipitate is observed, it should be replaced immediately. Store in a sealed container to protect from air and moisture. It is advisable to maintain the storage temperature between 4°C and 30°C, which will help retain the drug's efficacy and safety for a longer duration.
