Improvement of essential hypertension
Essential hypertension helps lower blood pressure to normal levels, reducing the risk of heart and vascular diseases. This medication contributes to the prevention of complications through blood pressure control and improves the overall health status of patients. It is primarily used for the treatment of essential hypertension.
The recommended initial dose is 10 mg once daily, with or without food. If blood pressure is not adequately controlled, it can be increased to 20 mg once daily. If further blood pressure reduction is required, the maximum dose can be increased to 40 mg once daily. Blood pressure-lowering effects generally appear within 2 weeks of starting treatment, with maximum efficacy achieved in approximately 8 weeks. If blood pressure is not controlled with this medication alone, thiazide diuretics such as hydrochlorothiazide can be added. For compliance improvement, taking the medication around the same time each day (e.g., in the morning) is recommended. In patients who may have a reduction in effective intravascular blood volume (e.g., those on diuretics, especially those with renal impairment), it is advisable to observe the condition closely and start treatment at a lower dose. <For the Elderly> There is no need to adjust the dosage when administering this medication to elderly patients aged 65 and older. No overall differences in efficacy and safety between elderly and younger patients have been observed, but it cannot be excluded that some elderly patients may respond more sensitively. <For Patients with Renal Impairment> The maximum dose for patients with mild to moderate renal impairment (creatinine clearance of 20-60 mL/min) is 20 mg once daily. <For Patients with Liver Impairment> Patients with mild liver impairment do not require dosage adjustments. The recommended initial dose for patients with moderate liver impairment is 10 mg once daily, with a maximum of 20 mg per day. There is no experience in using this medication in patients with severe liver impairment, so its use is not recommended. <For Children> The safety and efficacy in children have not been established.
The safety and efficacy in children have not been established.
This medication can cause serious harm to the fetus and newborn if taken during the second and third trimesters of pregnancy, so it should be discontinued immediately upon confirming pregnancy. Patients with allergies to this medication or certain genetic lactose intolerance diseases, pregnant or nursing women, patients undergoing kidney dialysis, patients with severe liver dysfunction, and patients with bile duct obstruction should not take this medication. Patients with reduced blood volume or sodium, impaired kidney function, renal artery hypertension, or at risk of hyperkalemia should use this medication with caution, and their blood pressure, kidney function, and serum potassium levels should be regularly monitored. Taking this medication may cause various side effects such as dizziness, headache, indigestion, muscle pain, and rash, but most are mild and transient. Rare side effects, such as chronic diarrhea accompanied by weight loss and intestinal vascular edema, have also been reported; immediate consultation with a doctor is necessary if such symptoms occur. Taking this drug in conjunction with potassium-sparing diuretics, potassium supplements, lithium, non-steroidal anti-inflammatory pain relievers, etc., may increase the risk of side effects, so always consult a physician or pharmacist when using other medications together. This medication should not be taken during pregnancy due to the risk of fetal damage, and if nursing, breastfeeding may affect the baby, so either breastfeeding should be stopped or medication discontinued. The safety of this medication in children has not been established, so it is advisable not to use it in children. Older adults should be cautious as drug metabolism may vary. In the case of overdose, symptoms such as hypotension, palpitations, and bradycardia may occur; appropriate treatment should be sought immediately from a medical facility. No carcinogenic risk has been reported with long-term use, but any unusual symptoms should always be discussed with a healthcare professional.
