Summary

Main use
Improvement of essential hypertension
Who takes it
Adults · Children
Adult dose
The recommended initial dose for adults is 10mg once daily, administered regardless of meals. If blood pressure is not adequately controlled, the dose may be increased to 20mg once daily, and may be further increased to a maximum of 40mg per day if necessary. The antihypertensive effect appears within 2 weeks of starting treatment, with maximum effect occurring around 8 weeks. If blood pressure control is insufficient with monotherapy, thiazide diuretics such as hydrochlorothiazide may be used in combination. To improve adherence, it is recommended to take the medication at the same time each day (for example, in the morning). In patients who may have intravascular volume depletion (e.g., patients on diuretics, especially those with kidney impairment), it is advisable to start treatment at a lower dose while closely monitoring their condition. <Elderly> For elderly patients aged 65 and older, dose adjustment is not required. Although no overall differences in efficacy and safety have been observed between elderly and younger patients, some elderly patients may exhibit more sensitive responses. <Patients with Kidney Impairment> The maximum dose for patients with mild to moderate kidney impairment (creatinine clearance 20-60mL/min) is 20mg once daily. <Patients with Liver Impairment> No dose adjustment is needed for patients with mild liver impairment. The recommended initial dose for patients with moderate liver impairment is 10mg once daily, which can be increased to a maximum of 20mg daily. There is no experience in using this medication in patients with severe liver impairment, thus its administration is not recommended. <Children> Safety and efficacy in pediatric patients have not been established.

Precautions

This medication may cause serious harm to the fetus and newborn if taken during the second and third trimesters of pregnancy, and should be discontinued immediately upon confirmation of pregnancy. Patients with allergies to any component of this medication, as well as pregnant or breastfeeding women, patients on kidney dialysis, patients with severe liver impairment, and patients with biliary obstruction should not take this medication. Patients with lactose intolerance should avoid this medication due to its lactose content. Patients with low blood volume or sodium deficiency may experience low blood pressure upon the initial administration of this medication, so careful observation by healthcare professionals is necessary. Patients with severe heart failure or impaired kidney function may have worsening kidney function after taking this medication, requiring regular blood tests. Patients with kidney disease, those who have received kidney transplants, and patients with renovascular hypertension should be monitored for changes in kidney function when taking this medication. Patients at risk of hyperkalemia should frequently monitor their serum potassium levels while taking this medication, as hyperkalemia can occur. Patients with aortic stenosis, mitral stenosis, obstructive or hypertrophic cardiomyopathy should use this medication with special caution. Patients with primary aldosteronism may have a poor response to this medication, so its administration is not recommended. This medication lowers blood pressure, which may lead to dizziness; thus, it is advisable to avoid driving or operating dangerous machinery. Common side effects that may occur with this medication include dizziness, headaches, back pain, bronchitis, diarrhea, palpitations, and most are mild and temporary. Rarely, swelling may occur in the face or neck, and if any unusual symptoms appear, it is important to consult a healthcare professional immediately. This medication may affect liver function tests, so patients with liver disease should have regular liver function monitoring. Serious allergic reactions, muscle diseases, acute kidney failure, hair loss, itching, and other side effects have been reported rarely with this medication. Patients taking this medication over the long term should inform their healthcare professionals immediately if symptoms such as chronic diarrhea or weight loss occur, considering the possibility of gastrointestinal disorders. When taken in conjunction with potassium-sparing diuretics or potassium supplements, there is a risk of dangerously elevated serum potassium levels, so caution is warranted. When used with other antihypertensive medications, blood pressure may drop too low, so frequent blood pressure checks are required. Taking lithium alongside this medication may increase lithium levels, leading to toxicity; therefore, concomitant use is not recommended. Concomitant use with non-steroidal anti-inflammatory drugs may increase the risk of kidney function worsening, so caution is also needed in this circumstance. Pregnant women should avoid this medication, as it poses serious risks to the fetus; if planning a pregnancy or currently pregnant, consultation with a healthcare professional is essential. Breastfeeding women should discontinue breastfeeding or this medication due to the possibility of transfer into breast milk. Safety in pediatric patients has not been established, so it is not recommended for use in children. Elderly patients may have higher drug concentrations in the body, necessitating caution during administration.
