Improvement of essential hypertension
Essential hypertension is a condition that is very important for reducing the burden on the heart and blood vessels by controlling blood pressure within a normal range, and for preventing complications. This medication is used in the treatment of essential hypertension to lower blood pressure and improve cardiovascular health. Continuous use of this medication reduces the risk of stroke, myocardial infarction, and contributes to improving the overall quality of life of patients. This medication is used for the effective management of essential hypertension.
The recommended initial dose for adults is 10 mg once daily, taken regardless of meals. If blood pressure is not adequately controlled, the dose may be increased to 20 mg once daily, and if additional blood pressure reduction is needed, the dose may increase up to a maximum of 40 mg daily. Blood pressure-lowering effects begin within 2 weeks of treatment initiation, and maximum effects typically occur around 8 weeks. In cases where blood pressure control is not achieved with monotherapy, it may be used in combination with thiazide diuretics such as hydrochlorothiazide. To improve adherence, it is recommended to take the medication at the same time daily (e.g., in the morning). For patients with potential intravascular volume depletion (e.g., patients taking diuretics, especially those with renal impairment), treatment should be started at a lower dose while closely monitoring their condition.
Safety and efficacy for pediatric use have not been established.
This medication can seriously harm the fetus and newborn if taken during the second and third trimesters of pregnancy, so it must be discontinued immediately if pregnancy is confirmed. Do not take this medication if you have an allergy to any of its components, are pregnant, or are breastfeeding. It should not be administered to patients receiving kidney dialysis, those with severe liver disease, patients with biliary obstruction, or those with certain genetic lactase deficiency. Diabetic patients taking aliskiren formulations or those with moderate to severe renal impairment should not take this medication. Patients with reduced blood volume or salt may experience hypotension during the initial stages of treatment with this medication, so their condition should be carefully assessed, and appropriate measures should be taken if necessary. Patients with severe congestive heart failure or those whose kidney function depends on the renin-angiotensin-aldosterone system may experience changes in renal function, so caution is required. Patients with poor renal function should have their potassium and creatinine levels regularly checked by blood tests, and this medication is not recommended for patients with severe renal impairment. Patients with renal artery stenosis should be cautious about the risk of worsening kidney function with prolonged use. This medication may increase serum potassium levels, so patients with heart failure or renal impairment should frequently check their potassium levels. Patients with aortic or mitral valve stenosis, obstructive or hypertrophic cardiomyopathy should exercise special caution when using this medication. Patients with primary aldosteronism may not respond to this medication, and its use is not recommended. This medication has a blood pressure-lowering effect that may cause dizziness, so caution should be taken when driving or operating hazardous machinery. The effect of blood pressure lowering may be increased if this medication is used with other antihypertensive medications, so it should be discussed with a physician. Taking this medication with potassium-sparing diuretics or potassium supplements can lead to dangerously elevated potassium levels. Avoid using it with lithium as it may increase lithium levels and cause toxicity. Caution is advised when used with non-steroidal anti-inflammatory drugs (NSAIDs) as it may worsen kidney function. Taking this medication concurrently with angiotensin-converting enzyme inhibitors or aliskiren increases the risk of hypotension, hyperkalemia, and renal impairment, so co-administration should be avoided. This medication is minimally metabolized in the liver, leading to fewer interactions with other medications, but if abnormal symptoms occur during use, consult medical professionals immediately. If symptoms like chronic diarrhea, abdominal pain, or weight loss occur during treatment, inform the physician immediately as there may be a possibility of chronic malabsorption-like intestinal disorder. Rarely, angioedema or allergic reactions may occur after taking this medication, so if swelling of the face or neck or difficulty breathing appears, immediate emergency treatment is required. Taking this medication during pregnancy can result in severe problems such as renal impairment of the fetus, oligohydramnios, or craniofacial malformations, so it should never be taken during pregnancy. If breastfeeding, this medication may pass into breast milk and cause adverse reactions in the baby, so breastfeeding should be discontinued or this medication should be paused. Safety and efficacy for pediatric use have not been established, so it is advisable to avoid use in children. Elderly patients may have higher drug concentrations, so dosage adjustments may be necessary.
