Treatment of essential hypertension and reduction of cardiovascular disease risk
This medication is used to treat essential hypertension. Additionally, it helps lower the risk of major cardiovascular disease occurrence in high-risk patients aged 55 and older with a history of coronary artery disease, peripheral artery disease, stroke or transient ischemic attack, or organ damage due to diabetes. It reduces the risk of myocardial infarction, stroke, and death due to cardiovascular diseases. Therefore, it improves the management of hypertension while preventing cardiovascular diseases.
1. Essential Hypertension Telmisartan should be taken orally at a dose of 40 mg once daily with water, without regard to meals. Depending on the patient, a dose of 20 mg once daily may be effective, and if necessary, it may be increased to 80 mg once daily. When increasing the dose, the maximum antihypertensive effect is achieved after 4 to 8 weeks of treatment. This medication can be used in combination with thiazide diuretics such as hydrochlorothiazide. 2. Reduction of Cardiovascular Disease Risk Adults: It is recommended to take this medication orally at a dose of 80 mg once daily with water, without regard to meals. The risk reduction effects on morbidity and mortality related to cardiovascular diseases at doses below 80 mg are not known. When initiating treatment with this medication for the purpose of reducing cardiovascular disease risk, it is advisable to monitor blood pressure, and appropriate adjustments of antihypertensive treatment may be required. ○ Patients with Liver Impairment Caution should be exercised in patients with mild to moderate liver impairment, and the dose should not exceed 40 mg once daily.
This medication can cause serious harm, especially to the fetus and newborn, during pregnancy, so it should be discontinued immediately if pregnancy is confirmed. It should not be used in individuals with hypersensitivity to this medication, pregnant or breastfeeding women, patients with severe liver impairment or biliary obstruction, patients with hereditary angioedema, and certain renal impairment patients. Caution is advised for the elderly, patients with hyperkalemia, mild to moderate liver impairment, valvular heart disease, ischemic heart disease, gastrointestinal disease, renovascular hypertension, and patients with severe renal impairment. During treatment with this medication, various adverse reactions may occur, and symptoms such as hypotension, changes in renal function, hyperkalemia, angioedema, liver dysfunction, interstitial lung disease, and rhabdomyolysis should be closely monitored. Patients with decreased kidney function or kidney transplant recipients should regularly check serum potassium and creatinine levels, and those with reduced blood volume or sodium are at risk for hypotension and should correct their status before taking this medication. Caution is needed when taking this medication with other antihypertensive agents as it may enhance the antihypertensive effect, and interactions may occur when used with certain drugs, so consultation with healthcare professionals is essential. Pregnant women or those planning to become pregnant should not take this medication, and breastfeeding mothers should also avoid it. The safety and efficacy of this medication for children under 18 years have not been established; therefore, it is not recommended. Elderly patients are at risk of stroke due to excessive blood pressure reduction, so their condition should be monitored closely during treatment. In case of overdose, symptoms such as hypotension, dizziness, acute renal failure may occur, and emergency medical treatment should be sought immediately.
