Treatment of essential hypertension and reduction of cardiovascular disease risk
This medication is primarily used for patients with essential hypertension. It also helps reduce the risk of cardiovascular disease in high-risk diabetic patients aged 55 and older with a history of coronary artery disease, peripheral artery disease, stroke, transient ischemic attacks, or evidence of target organ damage. This drug is effective in reducing the risk of death related to cardiovascular events such as myocardial infarction and stroke. It can be administered tolerably to patients for whom ACE inhibitors are difficult to use. Therefore, it is effectively used to reduce the risk of essential hypertension and cardiovascular diseases.
For essential hypertension, adults should take 40 mg of telmisartan orally once daily with water. It can be taken regardless of meals. Depending on the patient, 20 mg once daily may also be effective, and if necessary, it can be increased to 80 mg once daily. An increase in dosage shows maximal antihypertensive effects 4 to 8 weeks after treatment initiation. For the reduction of cardiovascular disease risk, adults are recommended to take 80 mg of this medication orally once daily with water, regardless of meals. The risk reduction effects on morbidity and mortality of cardiovascular disease at doses below 80 mg are not known. When starting treatment with this medication for risk reduction of cardiovascular diseases, monitoring blood pressure is recommended, and proper adjustment of antihypertensive medication may be required. ○ Patients with liver impairment Caution should be exercised when administering to patients with mild to moderate liver impairment, and the dosage should not exceed 40 mg once daily.
This drug can cause serious harm to the fetus and newborn, especially during pregnancy, so it should be discontinued immediately if pregnancy is confirmed. This medication should not be taken by individuals who are allergic to it or its ingredients, pregnant or breastfeeding women, people with severe liver impairment or bile duct obstruction, patients with hereditary angioedema, and those with certain kidney diseases. Elderly patients, those with high serum potassium levels, individuals with mild to moderate liver impairment, heart valve disease, cardiovascular disease, gastrointestinal disease, renal artery hypertension, and patients with severely impaired kidney function should take this medication with caution. This medication may cause various adverse reactions such as hyperkalemia, renal dysfunction, hypotension, allergic reactions, and liver function abnormalities, so it is important to monitor symptoms during use and consult a doctor immediately if adverse symptoms occur. In particular, if symptoms such as a sudden drop in blood pressure, dizziness, fainting, dyspnea, and swelling of the face, lips, or tongue occur, immediate medical treatment should be sought. Patients with renal impairment or those who have undergone kidney transplantation should have their potassium and creatinine levels monitored regularly. In cases of low blood volume or sodium depletion, there is a risk of hypotension, so conditions should be corrected before taking this medication. When taken with other antihypertensive agents, the blood pressure-lowering effect may be increased, and there may be a risk of renal function deterioration or hyperkalemia when taken with specific drugs, so current medications must be reported to the doctor. Women who are pregnant or planning to become pregnant should not take this medication, and breastfeeding women should avoid taking it or interrupt breastfeeding. Children under 18 years should not use this medication as its safety and efficacy are not established. Elderly patients should take the medication carefully to prevent excessively low blood pressure and be aware of dizziness or fainting during use. In cases of overdose, symptoms such as hypotension, dizziness, and acute renal failure may occur, necessitating immediate medical attention for appropriate treatment. This medication should be stored out of reach of children and kept sealed until just before use to protect its quality. Caution should be exercised while driving or operating dangerous machinery as there may be a risk of dizziness or fainting due to hypotensive effects.
