Efficacy & effects

Improvement of nutritional status through supplementation of trace elements (zinc, copper, etc.)

View approved original text

Administering trace elements including zinc, copper, manganese, selenium, and chromium during intravenous nutritional supplementation helps maintain nutritional balance and prevents essential mineral deficiencies in patients. These trace elements are essential for various enzyme reactions and metabolic processes in the body and play important roles in enhancing immune function and wound healing. Therefore, appropriate supplementation of trace elements during intravenous nutrition contributes to the overall recovery and health improvement of patients.

How it is given

Adult administration

To be used as directed by a physician, adjusting dosage according to plasma trace element levels. For metabolically stable patients receiving parenteral nutrition, administer Zinc 2.5–4 mg per day, Copper 0.5–1.5 mg per day, Manganese 0.15–0.8 mg per day, Chromium 10–15 µg per day, Selenium 20–40 µg per day. For other metabolically stable patients with intestinal fluid loss, additional 12.2 mg/L (intestinal fluid loss)

Child administration

In pediatric patients under 5 years of age: Zinc 100 µg per kg of body weight per day, Copper 20 µg per kg of body weight per day, Manganese 2–10 µg per kg of body weight per day, Chromium 0.14–0.20 µg per kg of body weight per day, Selenium

Precautions

This medication should not be administered to premature infants and newborns as it contains benzyl alcohol, which can cause fatal respiratory issues in these populations. Injecting this medication directly into the vein without dilution may cause phlebitis, so it must always be diluted before use. Additionally, since this medication is a hypoosmolar solution, it should only be used for mixed injections. If any toxic symptoms occur due to any of the components during administration, the use must be stopped immediately. The aluminum present in parenteral nutrition solutions may reach toxic levels if administered long-term to patients with impaired renal function, particularly in premature infants. Before administration, the physician should carefully assess the patient's condition and metabolic needs and regularly check blood trace element levels for dosage adjustment. In patients with renal failure, zinc elimination can be difficult, leading to accumulation in the body, so caution is necessary. Patients with bile duct obstruction may have difficulty excreting copper and manganese, leading to accumulation of these trace elements. Chromium is primarily excreted through the kidneys, and for diabetic patients, the use of antidiabetic medications should be checked. Selenium dosage adjustments are needed for patients with kidney and gastrointestinal dysfunction, and for patients receiving transfusions, the impacts of transfusions should be considered. For pregnant women or women of childbearing potential, the administration of this medication should be decided with caution, and should only be used if the benefits outweigh the risks. Excessive doses can cause toxic symptoms of trace elements such as zinc, copper, chromium, and selenium, so the prescribed dosage must be strictly adhered to. Particularly, acute toxic symptoms may occur with excessive zinc and symptoms such as dehydration, diarrhea, and hypotension may occur with excessive copper. This medication should not be used without a physician's prescription, and it is important to maintain aseptic conditions when adding to parenteral nutrition solutions. The solution should be inspected for discoloration or particles before administration. When storing, place the medication in a sealed container to protect it from air or moisture. It is recommended to maintain a storage temperature between 15 and 30 degrees Celsius (room temperature). Proper storage will help preserve the medication's effectiveness and safety.
