Summary

Main use
Treatment of obesity (weight loss and maintenance); reduction of the risk of weight regain
Who takes it
Adults · Children
Adult dose
Adults The recommended dose is 120 mg of orlistat per dose, 3 times daily by mouth, taken with a fat-containing meal or within 1 hour after a meal. Patients should continue a nutritionally balanced, mildly hypocaloric diet in which approximately 30 % of calories are derived from fat. Daily intake of fat, carbohydrate, and protein should be distributed evenly among the three meals. This medicine may be omitted if a meal is skipped or if the meal contains no fat. Because this medicine may reduce the absorption of some fat-soluble vitamins and beta-carotene, patients are advised to take a multivitamin containing fat-soluble vitamins to ensure adequate nutritional intake. The vitamin supplement should be taken at least 2 hours before or at least 2 hours after this medicine, such as at bedtime. Taking more than 120 mg per dose, 3 times daily, does not increase the efficacy of this medicine. Based on measurements of fecal fat, the effect of this medicine appears approximately 24 ∼ 48 hours after administration. After discontinuation, fecal fat content usually returns to pretreatment levels within 48 ∼ 72 hours. The efficacy and safety of this medicine for periods exceeding 4 years have not been evaluated. Studies have not been conducted in patients with hepatic and/or renal impairment or in elderly patients.
Child dose
Studies have not been conducted in children younger than 12 years.

Precautions

This medication should not be used in patients with chronic malabsorption syndrome or complete biliary obstruction, and in those with allergies to this medication or its components. Women who are pregnant or may become pregnant, and those who are breastfeeding should also not take this medication. If there are any signs of liver damage or abnormalities in liver function, patients should consult a doctor immediately, and discontinuation of this medication may be necessary. Additionally, patients with hyperoxaluria or calcium oxalate kidney stones should use this medication with caution. Adverse effects predominantly manifest as various gastrointestinal symptoms, which are related to the medication's action mechanism. Commonly reported symptoms include abdominal pain, abdominal bloating, flatulence, constipation, fatty stools, increased bowel movements, or fecal incontinence; these symptoms are usually mild and temporary. However, some patients may experience symptoms lasting more than 6 months, requiring attention. Discontinuation of medication due to adverse reactions commonly occurs, primarily due to gastrointestinal adverse reactions. If symptoms of liver dysfunction such as loss of appetite, itching, jaundice, changes in urine color, or upper right abdominal pain occur, patients should inform their doctor immediately. Taking this medication with a high-fat meal may exacerbate gastrointestinal adverse reactions, so it is recommended to spread fat intake equally across three meals a day. This medication may interfere with the absorption of fat-soluble vitamins and beta-carotene, so taking vitamin supplements is advisable, adjusting the timing to either at least 2 hours before or after taking this medication. Due to weight loss, diabetic patients may need adjustments in their blood sugar control medications. Excessive weight loss may increase the risk of gallstones, so caution is advised. This medication may interact with other drugs; therefore, patients taking cyclosporine, anticoagulants, antiepileptic drugs, or antiretrovirals should consult their physician. Use during pregnancy is not recommended, and it should not be taken during breastfeeding. It is not intended for use in children under the age of 12 due to established safety and efficacy. In case of overdose, careful monitoring is required, and adverse reactions generally resolve quickly. This medication should be kept out of the reach of children and stored protected from light and moisture.
