Treatment of essential hypertension and heart failure (intolerance to ACE inhibitors), reduction of mortality risk after myocardial infarction
This medication is used to effectively control blood pressure in patients with essential hypertension. Additionally, it alleviates symptoms and supports cardiac function in heart failure patients who are intolerant to ACE inhibitors, specifically those classified as NYHA class II to IV. Furthermore, it plays a crucial role in reducing the risk of mortality in patients with left ventricular dysfunction following myocardial infarction. Due to these efficacies, this medication is used for hypertension, heart failure, and risk reduction after myocardial infarction.
In essential hypertension, a recommended starting dose is 80 mg orally once daily as valsartan. If blood pressure control is inadequate, the dose may be increased up to 160 mg daily, with a maximum dose of 320 mg possible. For elderly patients (75 years and older), a starting dose of 40 mg once daily is recommended. For patients with renal impairment, those with a creatinine clearance of 10-20 mL/min should start at 40 mg once daily.
Safety and efficacy have not been established in children and adolescents under 18 years.
This medication can cause severe harm, particularly to the fetus and newborn, during pregnancy, so it must be discontinued immediately if pregnancy is confirmed. Pregnant women, women who may become pregnant, and nursing mothers should not take this medication. This medication is not to be used in patients with severe liver dysfunction, biliary obstruction, or cholestasis. Patients with hypersensitivity or a history of angioedema should also refrain from taking this medication. Its use is not recommended for patients with severely impaired renal function or certain kidney diseases. Elderly patients or those with elevated serum potassium levels or mild liver function impairment should use this medication with caution. Patients with valvular heart disease or cardiovascular diseases also require careful monitoring during administration. When taking this medication for the first time, blood pressure may drop suddenly; therefore, especially for patients with fluid or sodium depletion, it is important to start with a low dose and closely monitor the condition. As this medication lowers blood pressure, caution should be exercised for activities such as driving or operating machinery where dizziness or drowsiness may occur. It is important to regularly check kidney function and potassium levels through blood tests while taking this medication. Interactions may occur if used with other antihypertensives, diuretics, or potassium-sparing medications, so consultation with a physician is necessary. This medication contains certain pigment components, so caution is required for those with a history of allergies. In case of overdose, severe hypotension or consciousness impairment may occur, necessitating immediate medical assistance. Safety for children and adolescents has not been established, so this medication is not recommended for use in those populations. This medication should be stored out of reach of children and should not be transferred to other containers to ensure safety.
