Precautions

This medication is used as a local anesthetic and may cause a rare side effect known as methemoglobinemia. Methemoglobinemia is a condition where the oxygen-carrying capacity of the blood is decreased, which may result in symptoms such as cyanosis of the skin or odd blood color. Extra caution is needed for patients with glucose-6-phosphate dehydrogenase deficiency, congenital methemoglobinemia, heart or lung disease, infants under 6 months of age, or when used with oxidizers. Symptoms should be closely monitored, and if severe, administration should be stopped immediately and treated. This medication should not be used in patients with hypersensitivity to the ingredients, on areas with wounds or dermatitis, in infants under 3 months of age, in infants under 12 months of age taking medications that can induce methemoglobinemia, or in premature infants. Furthermore, caution is advised for patients with anemia or liver disease, elderly or frail patients, and those allergic to propylene glycol. When applied to the skin, temporary pallor, redness, swelling, itching, or burning sensation may occur, and rare cases of severe allergic reactions or anaphylactic shock may arise. It should not be used near the eyes, as it can cause irritation, and if it gets into the eyes, it should be flushed immediately with clean water. When used with other local anesthetics, certain anticancer drugs, antibiotics, antimalarials, or anticonvulsants, there may be an increased risk of methemoglobinemia, so caution is needed with combinations. Additionally, when used with antiarrhythmic agents, it may affect the heart, necessitating monitoring by healthcare professionals. When used by pregnant or nursing individuals, the potential effects on the fetus or newborn should be considered, and it must be used under the supervision of a physician. Special care is needed during breastfeeding, as the drug may be secreted in breast milk. For children, safety has been confirmed for single applications in infants from 3 months to 12 months, but application to the genital mucosa is not recommended, and it should not be used in conjunction with drugs that induce methemoglobinemia. In case of overdose, symptoms of methemoglobinemia or central nervous system and cardiovascular toxicity may occur, requiring immediate treatment, and patients should be closely monitored for several hours post-administration. This medication’s systemic absorption may vary depending on skin thickness, application site, and application duration, so caution is advised during use. Moreover, prolonged high-dose use should be avoided as it may increase carcinogenic potential.
