Summary

Main use
Treatment of obesity (weight loss and maintenance); prevention of weight regain
Who takes it
Adults · Children
Adult dose
The recommended dose is 120 mg of orlistat orally 3 times daily, taken with a meal containing fat or within 1 hour after a meal. Patients should continue to follow a nutritionally balanced, mildly hypocaloric diet in which approximately 30 % of calories are derived from fat. Daily intake of fat, carbohydrates, and protein should be distributed evenly among the three meals. This medicine may be omitted if a meal is skipped or if the meal contains no fat. Because this medicine may reduce the absorption of some fat-soluble vitamins and beta-carotene, patients are advised to take a multivitamin containing fat-soluble vitamins to ensure adequate nutrition. The vitamin supplement should be taken at least 2 hours before this medicine or at least 2 hours after it, such as at bedtime. Taking more than 120 mg of this medicine per dose, 3 times daily, does not increase its efficacy. Based on measurements of fecal fat, the effect of this medicine appears approximately 24 ∼ 48 hours after administration. After treatment is discontinued, fecal fat content generally returns to pretreatment levels within 48 ∼ 72 hours. The efficacy and safety of this medicine for periods exceeding 4 years have not been evaluated. No studies have been conducted in patients with hepatic and/or renal impairment or in elderly patients.
Child dose
Children under 12 years of age: No studies have been conducted.

Precautions

This medication should not be used in patients with chronic malabsorption syndrome or biliary obstruction. It should also not be taken by individuals with allergies to the drug or any of its ingredients, pregnant women, women who may become pregnant, or breastfeeding mothers. Patients with liver function problems should use this medication with caution. If symptoms of liver dysfunction occur during treatment, a doctor should be notified immediately, and liver function tests should be performed. Patients with hyperoxaluria or calcium oxalate kidney stones should also use caution. Adverse reactions mainly appear in the gastrointestinal system and can include abdominal distension, flatulence, constipation, and oily stools. These symptoms are generally mild and temporary, usually occurring within three months after beginning treatment and diminishing over time. However, in some patients, gastrointestinal adverse effects may persist for more than six months. Rare side effects such as liver damage, hypersensitivity reactions, pancreatitis, hypoglycemia, and seizures have also been reported during treatment. If allergy symptoms such as skin rash, itching, or difficulty breathing occur, medical assistance should be sought immediately. This medication can reduce the absorption of fat-soluble vitamins and beta-carotene, so it is advisable to take vitamin supplements, which should be taken at least 2 hours before or after taking this medication. Taking this medication with a high-fat meal can increase gastrointestinal adverse reactions, so it is important to distribute total fat intake evenly across three meals. Extreme weight loss can also increase the risk of gallstones, so caution is advised. This medication may interact with other medications, especially cyclosporine, anticoagulants, antiepileptics, and antiretrovirals, so it should be taken under the supervision of a doctor, along with close monitoring through blood tests. Pregnant and breastfeeding women should not take this medication, and it has not been confirmed safe for children under 12 years. In the case of overdose, no specific serious adverse reactions have been reported, but observation by medical personnel is necessary. Finally, this medication should be stored in a place out of reach of children, and kept away from light and moisture.
