Treatment of hypertension and kidney disease in patients with type 2 diabetes
This medication is used in the treatment of patients with hypertension and kidney disease due to hypertension in patients with type 2 diabetes.
For the treatment of hypertension, the initial and maintenance dose is 50 mg of losartan potassium administered orally once daily. If the therapeutic effect is insufficient, the dosage may be increased to a maximum of 100 mg once daily or divided into two doses. For kidney disease in patients with type 2 diabetes having hypertension, the usual dose is 50 mg once daily, and this may be increased to 100 mg depending on blood pressure. An initial dose of 25 mg is recommended for some cases. In elderly patients, particularly those over 75 years old, an initial dose of 25 mg is recommended.
For children and adolescents aged 6 years and older weighing between 20 kg and less than 50 kg, the recommended dose is 25 mg once daily, with a maximum increase to 50 mg. For those weighing 50 kg or more, the dose is 50 mg once daily.
This medication may cause serious harm to the fetus if taken during pregnancy, especially in the second and third trimesters; therefore, if pregnancy is confirmed, it should be discontinued immediately. During pregnancy, a decrease in amniotic fluid can inhibit fetal lung development and cause skeletal deformities, and risks of oliguria, hypotension, and renal failure may occur in newborns. Patients who are allergic to this medication, pregnant or potentially pregnant women, patients with severe liver impairment, and patients with certain genetic diseases should not take it. Additionally, patients with moderate to severe kidney impairment among diabetics should avoid combining this medication with specific drugs. Elderly patients, those with hyperkalemia, mild to moderate liver impairment, and patients with valvular heart disease should take this medication with caution, as blood pressure may drop suddenly, requiring careful monitoring. Side effects during treatment may include muscle cramps, back pain, dizziness, upper respiratory infections, and, rarely, serious adverse reactions such as angioedema, hepatitis, and muscle damage. Particularly, patients at risk of hyperkalemia may require regular blood tests due to potential elevations in serum potassium levels. This medication may affect liver and kidney function, so dose adjustments and caution are necessary in patients with liver or kidney diseases. If symptoms of dizziness or fainting occur due to excessively low blood pressure, immediate consultation with a healthcare provider is necessary. Furthermore, this medication may interact with specific drugs, so patients should inform their healthcare provider about any other medications they are taking. Women who are pregnant or breastfeeding should avoid taking this medication, and if it is unavoidable, breastfeeding should be discontinued. The safety and efficacy of this medication have not been sufficiently established in children under 6 years of age or in pediatric patients with severely impaired renal function, and therefore, it is not administered. In cases of overdose, hypotension and heart rate changes may occur, requiring prompt emergency treatment. Patients taking this medication should consult their healthcare provider regarding pregnancy plans or potassium supplement usage, and the medication should be stored out of reach of children.
