Treatment of mild gastroesophageal reflux disease and recurrent reflux esophagitis; prevention of NSAID-induced gastroduodenal ulcers
This medication is used to treat mild gastroesophageal reflux disease and related symptoms such as heartburn, acid reflux, and dysphagia. It is also indicated for long-term maintenance therapy to prevent the recurrence of reflux esophagitis. For patients who must continuously take non-steroidal anti-inflammatory drugs (NSAIDs), it helps prevent gastric and duodenal ulcers. Thus, this medication is effectively used for the prevention and treatment of various gastrointestinal diseases.
1. Treatment of mild gastroesophageal reflux disease and related symptoms Administer pantoprazole 20 mg orally once daily before breakfast. Symptom relief generally occurs within 2 ∼ 4 weeks, and healing may usually be achieved with 4 weeks of treatment. If healing is not achieved, an additional 4 weeks of treatment may result in healing. 2. Long-term maintenance therapy to prevent recurrence of reflux esophagitis Administer pantoprazole 20 mg once daily before breakfast. If recurrence occurs, increase the dose to pantoprazole 40 mg daily, and after healing, reduce it again to 20 mg daily. 3. Prevention of gastroduodenal ulcers caused by nonsteroidal anti-inflammatory drugs in patients requiring continuous treatment with nonsteroidal anti-inflammatory drugs Administer pantoprazole 20mg once daily. Dose adjustment is not required in elderly patients or patients with renal impairment. In patients with severe hepatic impairment, do not exceed 20 mg of this drug once daily.
This medication should not be used in certain patients. For example, individuals with an allergy to this medication or any of its components, or those who have experienced hypersensitivity reactions, should not take it. Additionally, patients currently taking certain antiviral medications such as atazanavir and nelfinavir should not use this medication. Women in early pregnancy or those who may become pregnant, as well as nursing mothers, are also advised against using this medication. Patients taking medications containing rilpivirine should not use this drug in conjunction. While taking this medication, various side effects may occur. Gastrointestinal symptoms such as abdominal pain, constipation, diarrhea, nausea, vomiting, and dry mouth are common. Systemic symptoms such as fatigue, lethargy, edema, and fever have also been reported. Liver function abnormalities may occur, so periodic monitoring of liver enzyme levels is necessary. Severe allergic reactions, skin rashes, hives, and serious skin disorders may occur infrequently, warranting caution. Additionally, symptoms such as muscle pain, joint pain, and neurological symptoms including headache, dizziness, and vision disturbances may occur. Mental health effects such as insomnia, depression, and hallucinations could also manifest. Various other adverse reactions including hematological abnormalities, metabolic issues, and kidney problems have been reported. This medication can mask malignant tumor symptoms or delay diagnosis; hence, if symptoms of gastric ulcers are present along with weight loss, recurrent vomiting, difficulty swallowing, hematemesis, or black stools, it is essential to confirm the malignancy. Regular check-ups during prolonged usage (especially for more than one year) are important. Patients with severe liver dysfunction may need to limit the daily dosage and undergo liver function tests during treatment. It is recommended not to use this medication for mild abdominal or intestinal pain. Long-term use of this medication may result in vitamin B12 absorption disorders, so caution is needed. Patients on continuous NSAID therapy may have an increased risk of gastrointestinal bleeding, necessitating careful administration. Additionally, prolonged high-dose use may increase the risk of hip, wrist, and spinal fractures. Cases of hypomagnesemia may occur rarely when used for more than three months and more frequently after one year of use. Hypomagnesemia can lead to severe arrhythmias or seizures, necessitating magnesium supplementation and discontinuation of the drug if needed. This medication suppresses gastric acid secretion, which can increase the number of bacteria in the stomach, raising the risk of infections such as salmonella, campylobacter, and Clostridium difficile. In particular, persistent diarrhea in hospitalized patients should raise suspicion of Clostridium infection. If dizziness or visual disturbances occur during use, activities requiring alertness such as driving or operating machinery should be avoided. This medication is metabolized by certain enzymes (CYP-450) in the liver and may interact with other drugs metabolized by the liver. In particular, bleeding risks may increase when taken with anticoagulants such as coumarins, necessitating frequent blood coagulation tests. Additionally, concurrent use with certain antiviral medications may reduce drug concentration and therapeutic efficacy. It should not be used during the first three months of pregnancy, and when used during pregnancy, it should only be used when its benefits outweigh the risks. Nursing mothers should avoid using this medication as it is excreted in breast milk. There is insufficient experience to recommend use in children under 12 years old. No specific symptoms of overdose are known, but if symptoms occur, symptomatic treatment should be provided. This medication should be taken whole and not chewed or crushed. The safety of long-term use is not fully established, so the benefits and risks should be carefully considered for use beyond one year. Keep this medication out of reach of children, and do not transfer it to other containers to maintain its quality.
